Utah Student Video Contest Submission Form

Before submitting the link to your video submission for the 2022 Student Video Contest, please make sure you've read the contest rules, content guideline, and reviewed the timeline of events.

BY SUBMITTING THIS FORM, YOU ARE AGREEING TO THE CONTENTS OF OUR ELIGIBILITY, TERMS, AND WAIVERS PAGE.

Every student under 13 whose information will be listed here must get a parent or guardian to sign and send BBB the Parental Consent Form (for students under 13 years old).

TEAMS: Remember that every student must be from the same school. Everyone must be a high school student that goes to a private or public school listed on the Utah.gov directory.

Please list how you will divide the money between your teammates if you win first or second place. If you will divide the prize equally, leave blank. 1st place receives $500 to the student team; 2nd place is $250.

By agreeing to the terms and conditions, I verify that I am the legal parent or guardian of this student participant in BBB Student Video Contest. I further warrant that my child would like to participate in the contest. I also hereby agree to provide express written permission for my child, if aged 13 or under, to participate in BBB Student Video Contest by filling out, signing, and sending back (through email, fax, or mail) a parental consent form.
