I was told that presenting my insurance card and drivers license was not sufficient for a basic teeth cleaning appointment. Unless I provided complete social security numbers, I would be required to pay 100% of the **** until **************** received payment from my dental insurance carrier. I explained I was the victim of identity theft and could not give my full social. I paid out of pocket for 2 of my family members. The day before my scheduled appointment I received a call from ****************** office stating I must provide my social or she would not see me. I reluctantly gave it. 4 months later the office called and asked for my social again. I told them I already provided it 4 months ago. The staff person apologized & said they must have lost it! They also said the reason I had not been reimbursed for my payments was because my carrier had not processed the claims yet. I logged on to my account and confirmed they had been processed within the same month as the appointments and funds had been deposited into ****************** bank account! I called **************** to request my reimbursement and was treated unprofessionally and told I created the problem because I did not give my full social! She also stated all of the delays were due to my poor choice of insurance carrier. I told her I had a 3 way call with her insurance staff member, my insurance carrier and me. After my carrier provided her payment confirmations, ****************** employee acknowledged the office has owed me a reimbursement for several months. My insurance carrier also notified her office employee that they had sent multiple requests to their office as they submitted incomplete claim information, which ****************** employee also confirmed their error. My insurance carrier stated they do not require a full social to process a claim if the members ID and member card is given to the dental office. This confirmed to me ****************** office lied to me as I never needed to provide additional information! A week later I received a check and a certified letter stating **************** would no longer see me or any of my family members ever and all appointments for my family and I were cancelled.
