            On or near 4-1-2021 I went to The Montclair Medical Center Emergency room with a Broken Clavicle. After a few months I never received a bill, nor did I receive any form of electronic communication regarding billing or an invoice for care, I have contacted the hospital 4 times for a bill (************) and never got a callback, I have contacted prime healthcare 4 times (************) with no callback, I also contacted patient relations two times (************) with no callback. Two days ago I received an itemized service sheet but it contains no payment information and to be completely honest it looks like it was printed off an old dot matrix printer (see attachments). My insurance has sent me payment and I would like help in accessing the patient portal to pay for the service I received. or obtain a detailed invoice with payment information.
