            This complaint is about a vertical platform wheelchair lift that was installed at ************************ ***************************************** This is the address listed in Step 1. I am *************************** and is a trustee at ************************ #1.The lift was installed at the church in April 2023, and was inspected and given a Certificate of Operation on April 11, 2023, by the ************** Department of *******************************, Unit # *******.The problem is that when the lift was first used on Sunday, April 16, 2023, the front end of the wheelchair that was entering the lift raised up and almost tilt over backward. The chair was caught by a bystander. I have contacted American Elevator several times and will download that information. I also contacted the administration that inspected the lift, OSHA, **** and FTC, I did include the loading ramp rise and *** suggestions of what the rise should be and provided pictures showing what the rise is.I know this unit has a Certificate of Operation, but it is unsafe as it is and I don't want anyone to get hurt, especially those who are already restricted.Plaese see the attachments.
