              I went to see my neurologist on April 10th. I asked my doctor to fill out my FLMA paper work due to the health condition. I paid the fee and my FLMA paper work was sent to Data Dimensions on April 12 to be completed. I hadnt received anything after two weeks. I called my doctors office and they gave me a number for the company. I left a message with Data Dimesions and never received a call back. On May 1st I tried to call again and the number was disconnected. I called my doctors office back and got two new numbers. I left a message for **** at ************ and ************. I explained in my messages that the paperwork was due on May 1st and needed a update so I could follow up with the leave team at my work so I wont lose my job. I called again on May 2nd and left messages at both numbers again. I also called the ************** and asked to speak to ***** manager or anyone else who was in charge. I was told to call back in 20 min.
