Complaint: I went to Aspen Dental for dental....quoted me a price and had me apply for a credit limit thru ******* credit to have my services completed. they charged all of my services prior to doing the service. I have not been back to get the services completed because I have had trouble with all the dental work they provided. So now I am sitting on a debt that I do not owe for because they fraudulently charged all the services prior to them being performed. Now I have ******* hounding me for their money and it has now affected my credit due to these fraudulently charges

Desired Settlement: I would like for ASpen Dental to refund me for the charges they charged to the credit card thru ******* so that I may pay off this debt and restore my credit.......they are poor poor business people they will not refund on their own accord so I need the BBB to assist. Thanks

Business Response:

Dear Ms. *******:

Thank you for sharing your concerns about the treatment you received.  I am writing to you on behalf of the Auburn, NY Aspen Dental branded practice, owned and operated by Aspen Dental of Central New York PLLC.  I certainly do apologize for your experience at the Auburn, NY Aspen Dental office.

I am in receipt of your complaint filed with the Better Business Bureau.  I have reviewed it and your Aspen Dental account.  With the exception of the fillings on teeth # 28 & 29, all agreed upon dental treatment was provided.  On April 07, 2016, a refund in the amount of $ 87.52 was processed back to ******* Finance.  Today an additional refund amount of $79.50 is being sent to Genesis.  Please allow 3-5 business days for receipt and posting.  That amount was applied to your initial exam and x-ray services as a result of your insurance not covering them at 100% as estimated.  The total amount refunded to ******* will be $ 167.02, which is actually more than was estimated for the fillings you did not receive.  For your review, enclosed is a copy of your Aspen Dental account ledger showing all transactions to date.

I appreciate you giving me the opportunity to respond to your inquiry.  A copy of this letter has been forwarded to the Better Business Bureau in response to your complaint.  I am available for any further questions at the below contact information.

Sincerely,

Complaint: I went to this location today as a returning patient with the complaint of a painful tooth. X-rays were taken and then Dr M****** came in for an exam where he did not find anything wrong. He recommended an antibiotic course as thought maybe there was an underlying infection where a piece of skin was overgrown. Okay fine. As I was at the front desk getting ready to leave, I asked about scheduling a cleaning for another day. The office manager said "oh they didn't take measurements?". She goes out back then asks me to wait for hygienist i n waiting room. Hygienist takes me back and tells me her name (Elenia) then says she needs to take measurements. I agree even though I am not sure the purpose. So she examines my teeth and calls out series of #'s. Then tells me that she has discovered " pockets " in some of my gums that are graded on a scale. The higher the # the worse the pocket and your incidence of periodontal gum disease. Has me sign a paper that she told me that. Okay fine. I go back up to office manager. She explains their treatment protocol which includes 3 different treatments over several months and aggressive home care with their products. This treatment costs almost $2000 with my insurance participating. So since I haven't had my teeth cleaned in a couple years, I want just a regular cleaning first with a script for clorahexadine rinse and then another eval after that. Office manager sasys "Oh, but our policy says that since we discovered the periodontal disease, we cannot give you a regular cleaning unless you buy the treatments." I feel like I am being denied basic treatment because I am not buying their expensive product. Everyone has a right to refuse right? I also feel like they should of explained what the measurments were for before I agreed to it. Does informed consent not apply in this case? Now my treatment will be delayed even more as I will now have to search for new dental care.

Desired Settlement: A regular cleaning, a script for Clorahexidine, and another eval of this periodontal gum disease in 6 months.

Complaint: I began my denture purchase at Aspen dentals Lockport, NY location. I was so poorly treated by the dentist there, that I switched to the Tonawanda location. They made me the top denture but is much too big. I was told tbey would make me a brand new one to fit better. The denture causes blisters in myy mouth. It will fall out if I do not use large amounts of adhesive. I am only 39 yrs old, so this is a very hard issue for me to deal with. Aspen has now made me a new denture but they will not give it to me. The dentist there is insisting that I purchase a lower partial or he will not give me the new upper denture. I understand that I will not have a perfect fit without having a partial or implants put in the bottom left side of my mouth, but after all I have been through with them, I do not want to purchase anything more from them. I have been spoken to rudely, hung up on & just plain dismissed by the dentisit & employees at Aspen dental. The upper denture is paid for & was incredibly expensive. Unfortunately, I realize now, no matter what Aspen makes a poor product. I cannot afford to have a whole new upper denture made elsewhere. The dentist is bullying me into purchasing something else to get what I have already paid for. Also, they are billing for the process to start a lower partial, but will not listen that I just want the top one they owe me. I am very stuck & in need of help. Thank you

Desired Settlement: I would ultimately like to be given back the money Aspen was paid to get them done somewhere else. The problem is they will take the top denture I have now & at my age I cannot go months without teeth. I think the only thing to do is cor them to give me the completed upper denture that is there. Also, for me to not be billed for anything associated with the lower partial. Dealing with all of this has been so horribly stressful & embarrassing for me.

Business Response:

Dear Ms. ******:

Thank you for sharing your concerns about the treatment you received.  I am writing to you on behalf of the Lockport, NY and  Tonawanda, NY Aspen Dental branded practices, owned and operated by Dental Services of Western NY PLLC.  I certainly do apologize for your experience at the Lockport and Tonawanda, NY Aspen Dental offices.

I am in receipt of your complaint filed with the Better Business Bureau.  I have reviewed it along with your Aspen Dental account.  On August 05, 2015, you met with the practice owner Dr. N***.  He informed you that remaking the full upper denture alone would not solve the issues you were having, and that you needed a partial lower denture as well.  In his clinical opinion it would not be in your best interest as a patient.  He did note in your chart records, that if you did not wish to proceed with treatment that he would authorize a refund of what you were charged for the full upper denture in November of 2013.  On October 21, 2105, you returned to the Tonawanda, NY office for treatment and had another impression done to begin the remake process.  You have not been seen as a patient since that date.  A return to clinic notice was mailed to you on November 28, 2015; however it was mailed to the address on file at that time of **** ******* **** **** ** ********, OH 44145.  Based on the information submitted on your BBB complaint I have updated your account to reflect the following address… ***** ***** ***** ******** ******, NY 14032.  If this address is “not” correct please let me know as soon as possible.

If you wish to proceed with treatment by accepting Dr. N***’s recommendation, or if you wish to discontinue treatment and receive a refund for the full upper denture, please contact Kathryn at the Tonawanda, NY Aspen Dental office at ###-###-#### (option 2).  Also, please note that the refund agreement requires that you sign the Denture Money Back Guarantee form, which acknowledges that you will return the denture in your possession within 90 days or you will be billed directly for the cost.  Your refund will be processed within 48 hours of receipt of the signed DMBG form.  Only the denture cost ($1,149.00) is eligible for refund.  Other services provided will not be refunded.  If you have any questions, please contact Kathryn and she will be happy to answer them for you.  For your review I am including with this correspondence a copy of your Aspen Dental account ledger history showing all transactions to date.

I appreciate you giving me the opportunity to respond to your inquiry.  A copy of this letter has been forwarded to the Better Business Bureau in response to your complaint.  I am available for any further questions at the below contact information.

Better Business Bureau:

I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

I was told upon 1st complaint to Aspen Dental regarding my current upper denture, that I would be made another denture to replace the one I have now, as it is too lage in my mouth & falls out easily, also making it very difficult to eat. I have gonr through the process with them of making a new upper denture, but they will not give it to me unless I make another purchase from them. I understand that it would be best for me to have a lower partial, but at this time I cannot afford to purchase one. That does not mean they do not owe me the upper denture that Aspen dental has made for me & will not give me unless I make that purchase of a lower partial. Aspen dental owes me the new upper denture they have regardless of me purchasing anthing further with them. I have exxplained to them that I understand about the new upper not being perfect without having the lower partial & I am willing to sign a document stating that. However, I want to be given my new upper denture that Aspen has. I do not appreciate being bullied into making a purchase I cannot afford at this time. I want to be given what I am owed.

Regards,

***** ******

Business Response:

Here is my response to the rejection.

____________________________________

In reviewing Ms. ******'s dental records again, both the process for re-making the full upper denture and the partial lower denture were started, but not completed.  There is no finished denture to deliver.  The original response to this complaint remains.  She can continue with the recommended treatment and receive a new Full Upper Denture at no charge along with a partial lower denture at a discount, or she can choose to received a 100% refund for the cost of the Full Upper Denture.  In either case Ms. ****** should contact the Tonawanda, NY Office Manager, Kathryn to discuss her options.

Complaint: I had mini implants done in September 2014, since then I have had problems with dentures not fitting right and broken bottom dentures. In the last 15 months I have had 3 broken bottom dentures. All the work has been done as warranty work due to the fact the dentures come with a 7 year warranty. At the rate I am going by the time the warranty runs out I will have had 21 new bottom plates. This to me is unacceptable and ludicrous. What is going to happen when the warranty expires and I have to pay for them myself because Aspen dental can't get it right?

Desired Settlement: Extended warranty on my dentures.

Additional Notes

Complaint: I went to Aspen Dental for dentures. One of the reasons I chose them was their money back guarantee. I got my immediate dentures in January 2015, then the permanent ones in October 2015. I've had nothing but problems since the beginning. I have never been even remotely happy with the service or product. They say the money back guarantee doesn't kick in until you get your permanents. What the don't tell you is that the only money you get back is on the permanents.....they called it an upgrade. Most of the money and all of the insurance was taken up by the immediates. I wrote them a letter on 12/15/15 sent to patientservices@aspendental.com. I received a call a couple weeks later from someone named Sabrina. She left a voicemail and I called back a couple times, but never reached her. I left her voicemail, but she never returned my call. I feel I have no other option than to file a complaint with the BBB. When I received my immediates, I needed many, many, many adjustments. I had blisters, sores, and bleeding on several occasions. Every time, I was treated like an inconvenience. Often, the dentist would say they couldn't see what I was talking about. I say "they" because I've seen four different dentists since January of this year. Two of them, Dr. Murray and a blonde woman dentist (never even got her name) were condescending and just wanted me to shut up, like I was complaining about nothing. One time, they were so bad that they actually popped a lymph node. I was told, if I ever had any problems, just come in and they would help. One day, I was in such pain that I could barely move my mouth to talk. I came in and was told that they couldn't see me. I told them about what they said about coming in anytime. She said yes, but any time they had an opening. She said it would be a week before they could see me. That was completely unacceptable and I insisted until they finally saw me. Neither the immediates or permanent dentures have fit right. One of the many problems I had with the immediates was they had a terrible under-bite. Dr. Murray told me that's how they have to be. I didn't believe that for a minute. When I went to try the wax bite for the permanent ones, they were far too big and loose. The dentist (the blonde) said when the permanent ones came back they would fit fine. Well, they didn't. They looked like huge horse teeth and floated around my mouth. She said I would just have to get used to them, because that's how it was. I could not accept that. I argued with her until she became exasperated and said "Fine! We'll do another impression." She was totally rude. I still wasn't satisfied with the finished product, but at that point, they said there was nothing else they could do for me. I knew I would have to file for a refund and find another dentist. These few examples are just the tip of the iceberg of the problems I've had with your company. I can't tell you how many times I left the office in tears. Now I'm told that I will only get the refund on the "upgrade" to the Comfilytes. Nothing else will be refunded to myself or my insurance. I'm in the process now of getting dental implants. Once I have the surgery, it will take 4 - 6 months of healing to place the new teeth. In the meanwhile, you wear your current dentures. I have to return my upgrades by 3/13/16. I'll be forced to wear the immediate dentures until I've healed. They don't even come close to fitting, so I'll have to endure another few months of torture. If Aspen Dental has any integrity and heart, they would allow me to extend returning the Comfilytes until I've completed the implant process. I went through a year of suffering and anguish, both mentally and physically, and feel that's the least they could do. My new dentist is prepared to send them a letter, including the treatment plan, if they need proof. We are hoping to do the surgery sometime between the end of February and mid-March. This process takes time, as well, and I began this process in Dec. 2015. As far as the dates complained on the next page....I don't have the exact dates, but it was almost every time I had an appointment from January to December. Too many times to count. I complained to the dentists, office manager, office staff, and Aspen's customer service.

Desired Settlement: I would like someone with authority from Aspen Dental to contact me. I would like someone to approve extending the return of my upgrades, in writing, until I've completed the implant process. As I mentioned in my complaint, I feel this is the least they could do after what I've been put through.

What I would like for Aspen Dental to resolve this issue, in lieu of their offer for the refund on the immediate dentures, is that I would still return the immediate dentures by 3/13/16 and they allow me to hang on to the Comfilytes until my implants heal to where I can get my new teeth.  At that point, I will also return those back to Aspen Dental.  I have been informed that the process takes 4 - 6 months and I am scheduled for surgery on 3/4/16.  I sent an email with the request to Mr. H**** after reviewing his response on Feb. 3 and have yet to hear back from him.  The immediate dentures don't fit at all.  They cut, rub, make blisters, and have a terrible underbite.  I wore them for 10 months, instead of the typical 6, because they couldn't get the permanent ones right.  I really can't stand the thought of wearing them for another 4 - 6 months.  The permanent ones still don't fit properly, but would at least be a much less painful option while the implants heal.  I would really like to resolve this issue as soon as possible.  I would not ask for any more of a refund and they would get both sets back.  One just later than the other.  Thank you for your consideration.

Regards,

****** *******

Additional Notes

Complaint: I scheduled a new patient appointment at Aspen dental in Brockton, Ma. my appointment was 12/30/15 and I was expecting a short routine appointment. After X-rays and examination, the hygienist and dentist told me I needed a deep cleaning, one new filling, and they wanted to replace two fillings the army did. I assumed this would happen in following appointments but they told me they would do it all that day. I've never had a fear of dentists and I trusted their opinion. My complaint begins with the deep cleaning. My entire mouth was numbed and she began the cleaning with no information on the pain I would be in after, the decreased ability to chew hours later, the antibiotic she injected into my gums (which I heard about after the fact). As soon as she finished the deep cleaning and stepped away, I looked at myself through my phone's camera and saw what appeared to be a tear on my lip. My glasses were dirty from the cleaning and I was a bit worn from the process, and then she rushed me over to the next room to get my fillings done. I had a minute to wipe off my glasses and looked at my lip again. The hygienist has drilled the skin off of my bottom lip and did not address it. The dental assistant who was going to work on my fillings with the dentist said, "did you bite your lip?" I said no, she drilled my skin off. The dentist then grabbed my some Vaseline for my lip and began my fillings. The fillings were done and I was send out the door with no instructions, paperwork, recovering tips, etc. Thankfully my boss allowed me to take the whole day off because I was in bad shape. I took pictures of my mouth as soon as I sat in my car. I took pictures days after showing my lip and how the skin was drilled off. To add, I told my mother about my appointment, as she has 25 years of experience in the dental field and this was my first appointment not at her place of work. She was shocked and called the office to ask about what happened. The receptionist divulged all of my personal appointment information to my mother without even asking who she was. Either way, their actions violated HIPPA laws. The hygienist told my mother that I must have accidently bit my lip. I know for a fact that I did not. I have complained and nobody is acknowledging me. I spent New Years eve indoors because of how my lip looked. I wish she would take responsibility and admit that she drilled the skin off of my lip.

Desired Settlement: I want the business to contact me and admit that they made several mistakes during my visit. I do not want to be charged for the deep cleaning that left me in pain for 6 days and left me embarrassed about my lip which is still not healed.

Complaint: I purchased the Comfilite package of dentures for my mom back in April 2015 as a gift because she needed them. During the initial price I paid, they stated that this package includes the price for the permanent denture. My mom has pegs and when it came down to the time that she was due to give them to her, the Office Manager told her that we had to pay an extra $2500 for her denture... I knew that this didn't sound right, long story short, she ended up getting a permanent denture that I initially paid for. She had the impressions done and when we went in to receive her permanent denture, the techician brought in these donald duck teeth and I said, those are not her teeth. The technician continued to tell her that those are her teeth and that they had her name on them. They tried to force them in her mouth but they didn't go in. Come to find out, they were SOMEONE ELSES teeth!!!! I was appauled!! They bring her teeth in and they have someone else's name on them that we demanded be fixed. My mom kept having to go back to CICERO from SHERRILL every week for 2 months, wasting tanks full of gas, for us to come back again and again. My mom and I went for what we thought was our final time, when we were told we had to come back up for her permanent soft realignement. Two weeks later, we received a bill stating that she owes $186 for this realignment. Again, this was and is included in the initial package that I paid for. After all the hassle we have gone through, I feel that my mom wasn't treated like she should be. Due to Aspen Dentals Money back Guarantee on dentures, I am requesting a full refund for what was paid for during the initial visit for her dentures.

Desired Settlement: I want a full refund for the dentures we paid for.

Complaint: Aspen wanted to grind on my wife's teeth to make their partial fit correctly. I told them we would seek a second opinion and went back to our original DDS. As he had advised us before we sought out a less expensive approach a bridge would be the only way to meet the requirements of her lost teeth issue. He also asked about the missing tooth that Aspen had extracted. His original diagnosis was to use the tooth Aspen extracted as an anchor tooth for a bridge on the other side of her mouth. His diagnosis was two different bridges would be necessary to make her bite correct. Aspen's opinion was to extract the anchor tooth rendering the possibility of a bridge on that side impossible. That tooth did not need to be extracted! We went back to our original DDS and are having a bridge placed on one side. The other side where the unnecessary extraction was is now only able to be fixed by an implant. We will wait and make a decision on the implant so a bridge can be placed on that side. Our understanding or rumor as it may be is Aspen has a quota for reaching a level of billing. So the extraction that was not necessary was simply an add on to increase the margin to cover their business cost/ profit. We did receive a $700 refund out of the $1900 bill but the additional approx. $4K will be greater than the original cost quoted from the original DDS to treat my wife's dental issues.

Desired Settlement: Full refund is the expected resolution due to the fact Aspen has cost us more money and pain and suffering than if we had stayed with the original DDS.

Complaint: Was billed $4256.50 as they did not even wait for the insurance portion to be applied. I changed mind and didn't have all the procedures done. My finial amount due from the invoices after insurance is $1648.40 . they said it was interest free for 1st six months but I was charged interest from the first amount billed. They finally refunded $2097.40 then $104 and now today they saw they will give me back another $296.00. that leaves a total amount charge as $1759.10. my total amount owed per insurance invoice is $1648.60.Leaving a total refund still owed as $150.50.//also on this treatment I was told I needed anti biotic for the gums so they did a $1011.00 worth of anti biotic which the insurance denied because it was deemed not needed because my condition didn't meet the criteria for such service..ie gums were not bleeding and the gums hadn't receded to the set number for worrenting the procedure. I trusted them to do what was needed and they lied to me.

Desired Settlement: I need the final $150.50 refunded . And I would like an adjustment on the unneeded gum treatment. And would like The $43 for the interest I have paid on the $4256.50 refunded.

Complaint: I had an appointment with Aspen Dental on 12/21/2015, When I arrived I was taken back for x-rays and an Exam, the person who assisted me thought that I was a new patient and notified me when I corrected her that they didn't have any of my records from previous times I went there.(EVEN THOUGH BACK IN 2009, The Apple Glen Location had a dentist that over drilled my tooth and had to crown it and eat the cost or I was suing the company. This is the only reason I remained with the company because nobody else would take me as a patient because they didn't want to be liable. If anything happens with this crown this company is responsible for messing up my tooth!) So, then a little later on a lady came back and said she thinks I was last there in 2010 and I informed her I was there in 2014, somehow my files were missing. But she assured me that I wouldn't be charged for todays exam and x-rays because I am a NEW PATIENT. After they got back there and did the exam Dr. Sitjar came back and began lecturing me that I have Peridontal disease and how I was going to loose my teeth and why my numbers went from 2 and 3s (When supposedly I DIDNT HAVE ANY RECORDS FROM THE LAST TIME I WAS THERE!!)to 23!-5's and a couple 6's. They told me that I could never be treated as a "normal" cleaning because I had periodontal disease and I was going to have to come back every 3 months to have maintenance cleanings and they were going to have to get in there under the gums and get out the infections and put antibiotic in there. I informed her that I was having no symptoms or pain in my mouth, she then stated that it is a SILENT disease and you never know when you have it. They then took me to the business office and the woman presented me with a treatment plan that was in the range of $1600.00-$2000.00 out of pocket and wanted a down payment before they could start anything. I was so angry, I went else where for a second opinion 2 days Later, and the new dentist told me that everything was great and there was no signs of any periodontal disease and they warned me of ASPEN Dental and how they Tend to SCAM people out of their money and they treat people as a Number not as an individual. On 1/22/2016 I got my insurance paper in the mail and I OWE ASPEN dental for a "TEST" it doesn't say what the charge was for but I was told I was not going to have NO charges that Day because I was a NEW PATIENT.

Desired Settlement: I SHOULDNT OWE ANYTHING BECAUSE THEY TOLD ME THAT I WAS A NEW PATIENT AND IT WAS A FREE EXAM. ON TOP OF THAT IT WAS NOT AN ACCURATE EXAM WHEN EVERYTHING THEY PUT IN THE CHART WAS A COMPLETE LIE ABOUT MY ORAL HEALTH.

Complaint: I recently went to Aspen Dental. After my appointment, I needed to give them my secondary insurance information. I called the location office I had recently been to. The receptionist told me that she could not help me, that she would have to send my insurance information to corporate to have them add it. This was Novemeber 1. I called back a week later to see if the secondary insurance had been added and she said no. It would take a lot longer. I explained that I needed the secondary insurance filled and would like the office manager to contact me. She took my name and number down but I never received a call from anyone. I called the Customer Complaint number I found online, they explained it had been updated and refilled December 15. I told them to have the office manager contact me. She never did. I called back 2 weeks later since my insurance company had no record of this visit being filled. They said it had been sent off to their insurance dept and that was all they could tell me. Again I asked to speak to the manager- no followup call. I called again a few days later and was told the paperwork had been sent off. I contacted my insurance company again and they still have not received it. This is beyond ridiculous for a business to treat their customers this way and lead them down this trail to nowhere. No one seems to care. I should not have to worry about following up with them to see if they did their job in a timely manner.

Desired Settlement: To have my previous visit filled with both insurances before my insurances expire.

Complaint: On May 12, 2015 I was seen at Aspen Dental for a broken tooth, to discuss IV sedation. I was taken for a full set of x-rays even though I told the assistant I did not want x-rays, but was told "All new Patients get x-rays, THEY ARE FREE." When the dentist, Dr. Kavita S***, saw me I explained I was only there to discuss possible IV sedation during possible repair of the tooth. Dentist told me they only use IV sedation for teeth extraction, not for very anxious patients. This dentist spent 2-3 minutes with me.I requested a copy of the x-rays that were done and the manager gave me a paper photo copy of the x-rays and told me there would be NO CHARGE for the visit. I was not given a copy of any charges or a bill.On May 15, 2015 my insurance processed a $208 bill for a comprehensive exam and complete x-ray images, and PAID $150.00On May 18, 2015 called Aspen Dental and Questioned why I was charged. A manager said "that was an error and your insurance will be reimbursed."On July 2,2015 I called, was told the issue goes through their corporate bill/refunds and could take 45 days to pay.I checked my insurance today, July 21, 2015, and no refund/reimbursement has been received.

Desired Settlement: Reimbursement of the $150 to my insurance, or to me, so I can use to pay my other dental bills.

Complaint: In October 2014, I set up several appointments with Aspen Dental. First was my wisdom teeth removal. I was called 4 hours prior to my surgery that I "should not have eaten after midnight the night before..." No one had called me and told me that, luckily I have enough knowledge to know that you cannot eat before surgery because the risk of aspirating. I was never given a consult about the procedure. The day I got there for my appointment, I was rushed to the back and immediately knocked out, no time for questions. I was shaking worried. Procedure went alright, despite being numb for over 12 hours afterwards. Although things seemed fishy about Aspen, I decided to schedule my next appointment to get Zoom Whitening, where I also had no consultation or warned about the pain I would go through. The procedure is 4 sets of 15 minutes under the light. During the second session, I started to feel pain, but I was left alone and had no way of telling anyone that I needed the whitening off immediately. So I sat there with excruciating nerve pain "zingers" until the machine beeped, and probably 3 minutes later the tech walked in. I tried to tell her I was done and it hurt too bad. She put "relief gel" on them which did nothing. I went home that night and did not sleep. The pain was so bad, nothing would make it go away. I left a review on the "Yelp" app on my phone about what happened, and the Regional Manager contacted me and refunded me. Despite the horrible experiences, I decided to go back, again. I made the appointment to get my veneers replaced on my front two teeth (8 & 9). I went back because the prices were a lot lower than anywhere else for the same procedure, and I could not afford to go anywhere else. When I went in to get my first veneers drilled off, I was sat in a chair and the doctor came back - she did NOT introduce herself, she immediately stuck a needle in my mouth and began giving me probably 20 shots to numb it. I was shaking because of the pain and nervousness. She drilled off my old veneers and began prepping my teeth for the temporary ones. In the process, she pushed my gums back making them uneven. She placed the temporary veneers quickly and I was out the door. These temporaries were a complete wack job - I realize they are temporary, you aren't supposed to bite with them, etc ... I have had temporary veneers before and they weren't this horrible. I was scared to get my permanent veneers, thinking the Doctor didn't have enough experience to make them look good, or even functional. But again, I went back because of the low prices. The day came for my permanent veneers, the color was alright, better than before (the sole reason I was replacing my old ones was because of the color.) She had trouble placing one of them, later I figured out why when I went to ******** Dental to get the Doctor to look at the horrible, ugly, wack job. ******** Dental was my primary dentist, but I couldn't afford their services. I was able to get in that day, Dr. Sexton at ******** Dental looked at my teeth and was extremely disappointed, almost laughing because of how horrible these veneers were. My left veneer (9) was NOT seated correctly, making gaps behind and to the side of my tooth, exposure which would result in cavities. Not only were they not seated correctly, but they do not even cover the sides of my teeth. Because of the exposure, I now have extreme nerve pain because my bare, drilled down, teeth underneath are exposed. Along with these problems, they look like complete crap. I contacted the Regional Manager (Scott Malarkey) to take action. He gave my his personal cell number, where we have called and texted about getting something done. I told him I wanted refunded, along with future services to fix my teeth paid for. I gave him appropriate documentation. He asked for a paper from ******** Dental stating "it was negligence of Aspen Dental" or else I probably would not get refunded, let alone next services covered. ******** Dental wrote a letter for me, but they did not want to get involved with legal matters so they didn't directly say what Scott said needed to be said for a refund. I told him I was not able to provide anything else for that reason. He said he would forward that to the new Regional Manager. The last I heard from him was Friday at 1:55PM, Feb 27th. It's been almost four months, and I am still paying $90 a month on my Dental First account for the $3600 worth of services done. I gross $20k a year, fresh out of college, I am 21 years old. All I wanted was to have some dental work done, and two veneers replaced solely because of the color. Now I wish I would have never done any of this. I'm in debt with this account, I want my teeth fixed and I cannot afford it. I want refunded totally for the horrible experience, along with my next veneers and gum work to correct the negligence of Aspen Dental paid for. I need help, I can't afford a lawyer, I don't know what else to do. Thank you so much,***** *********

Desired Settlement: I would like to be refunded the full amount I was charged by Aspen Dental, in the amount of $3639.57, as I had to have my teeth redone elsewhere.

Business Response:

Dear Ms. *********:

Thank you for sharing your concerns about the treatment you received.  I certainly do apologize for your experience at the Pickerington, OH Aspen Dental office.

I have received your complaint filed with the Better Business Bureau.  I have reviewed it and your Aspen Dental account with the Aspen Dental Territory Director and the Pickering, OH Aspen Dental Practice Owner.  The majority of your treatment at Aspen Dental encompasses two types of services.  You had wisdom teeth extracted in October 2014, and you had Veneers done in December 2014. As has been communicated to you in the past, in order to consider your request for a refund of the payments you made to Aspen Dental, we need to see documentation from another dental provider that supports your statements that corrective treatment needs to be done.  Upon receipt of that documentation we will review it and respond to you very quickly.  Please forward any documentation directly to my attention.

I appreciate you giving me the opportunity to respond to your inquiry.  A copy of this letter has been sent to the Better Business Bureau in reply to your complaint.  I am available for any further questions at the below contact information.

Consumer Response: Better Business Bureau:

I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

I provided documentation as much as I could. I sent pictures and proof from another dentist that they were placed incorrectly and would eventually need root canals. They (Sally P*****, Director of Op for Aspen Dental) stated that the letter from my dentist was not sufficient because he did not directly blame Aspen Dental. I cannot find a dentist who will do that because they say they do not want to get involved with legal matters.
This is absolutely ridiculous. I had to get them redone and pay another $3k. I cannot afford to pay for Aspen Dental's screw job.
As for the other services, their customer service is a joke. They didn't even send in info to my insurance company for my wisdom teeth. The company is a mess and I'm just beyond appalled they keep playing this game. I feel that I was baited into their crap services, now I'm stuck paying TWO dental bills so I don't ruin my credit. I will not give up until I get refunded. PLEASE HELP ME

A review of the patient's dental records show no copies of documentation submitted from another dental provider addressing her concerns and request to be refunded.  I do apologize for any inconvenience; however I will have to request that she re-sbumit any specific documentation from another dental provider to me.  She can fax it or e-mail it to me.  Upon receipt I will review with the clinical team and Field Management team and respond to her within a few days.  If the documents will be faxed, please make sure they are sent to my attention....(See contact information below)....

Complaint: I purchased dentures from this store and when I first went in I was told by ***** the office manager that after I got my teeth pulled that I was going to be leaving with temporary dentures and 6 months later to receive my permanents. Today when I went into the office I was told that I was only to get the one pair which is not what I was told originally on 10/15/14.

Desired Settlement: I would like to receive my second set of permanents like I was told by ***** that I would get.

Business Response:

Dear Mr. *****,

I am in receipt of your complaint filed with the Better Business Bureau.  I do apologize for your experience at the Pickerington, OH Aspen Dental office.

I have reviewed the complaint and your Aspen Dental account with the Regional Manager ****** *******, with whom your recently spoke to.  The dentures that you received and paid for in October 2014, come with a hard reline after the extraction healing process.  There is no second denture included.  I am very sorry for any misunderstanding in this matter.  ****** has advised me that she is working with you to resolve this.  Please follow up with her concerning this matter.

Thank you for allowing me the opportunity to respond to your inquiry.

Consumer Response: Better Business Bureau:

I have reviewed the response made by the business in reference to complaint ID 10569551, and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

Regards,

***** ******

I was offered dentues at half price. This is not what I was told when I first went in. My wife spoke to her co worker that referred us to them. She stated that she received to pair of dentures so why didnt I.  Their online add also states that the time difference between your temporary dentures and your permanent dentures will be different. My wife was also in the office when the employee ***** told me that even if i accidently broke my dentures that at least I will have a back up pair. This is false advertising at its best.

Business Response:

In response to this rejection, I have reviewed the account again.  I am very sorry for any misunderstanding, but Mr. ***** was not charged for the denture package that would have provided him a second set of dentures.  His agreement covered the initial immediate dentures at the time of extraction with a full hard reline after the healing process.   The denture package that provides two separate dentures would have cost him about $800 more.  The offer to accept 50% of the cost of upgrading was presented as a compromise and a courtesy.  I encourage him to reach out to the Regional Manager ****** to attempt to resolve this matter.

Sincerely,

Complaint: Approximately 06/13/2012, I had some teeth pulled and dentures made at Aspen Dental. From the beginning I told the dentist I needed soft, pliable dentures. I have been back about three times getting the dentures adjusted. It ok for a while then my mouth is sore again. 01. I sent an email to Aspen Dental 09/29/2014 asking them to remedy the situation. Email unanswered. 02. Sent US Mail letter 12/04/2014. Unanswered until approximately 1st week in January 2015 Mr.*****; phone number ###-###-#### contacted me, said he was a manager over this area looking into the matter and had to contact the doctor. 03. 01/31/2015 since I did not hear from Mr.*****, I texted him and made an inquiry as to why I had not heard from him. To date no response.

Desired Settlement: I want a pliable denture, (which I requested the first time).

Business Response:

Dear Mr. ******:

Thank you for sharing your concerns about the issues with your denture.  I certainly do apologize for your experience at the North Syracuse Aspen Dental office.

I have received your complaint filed with the Better Business Bureau.  I have reviewed it and your Aspen Dental account.  In June 2012 you had extractions done and received an immediate full lower denture and a replacement full upper denture.  In December 2012 you received a permanent reline on the full lower denture.  In February 2013, you received a denture adjustment at no charge.  The warranty on those dentures expired more than two years ago.

We will be happy to see you as a patient again; however no treatment will be provided without payment.  If you wish to make an appointment to be seen, please contact the Cicero, NY Aspen Dental office at ###-###-#### (option 2).

I appreciate you giving me the opportunity to respond to your inquiry.  A copy of this letter is being forwarded to the Better Business Bureau in response to your complaint.  I am available for any further questions at the below contact information.

Consumer Response:

Better Business Bureau:

I thank Mr. ***** for his response. However as I stated before I disagree greatly with his response. The following is why I disagree.

I told the dentist from the beginning I wanted pliable dentures, I explained to him I had delicate gums and hard acrylic dentures  would be uncomfortable for me. He took it upon himself to do just the opposite of what I requested him to do. I went back several times for adjustment, which eased the discomfort for a short time and then, the discomfort was back.  The fact that I kept going back was a clue that the dentures was not working for me. If the job was not what I requested, then the job was never completed and or under warranty. I requested pliable dentures, I received hard acrylic dentures. If the job was not done correctly according to what I requested, how can the dentures be out of warranty?  Aspen was paid from my care credit account approximately $ 2,100.00 of which I am still paying.

If you look at the length of  time  it Aspen Dental to respond to me, one can easily see why  the job would have ran out of warranty had it been done correctly.

Thank you very much.
I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

***** ****** Jr

Business Response:

Mr. ****** paid for and received full upper and full lower Basic dentures.  The warranty on those dentures was six months from the insert date.  Aspen Dental does not make a flexible "pliable" full denture.  We never have.  The only flexible dentures we make are partial dentures.  The $2,186.75 payment received from CareCredit in June 2012 covered the extractions and both dentures.  On September 20, 2012 a partial refund of $183.75 was sent back to CareCredit.  I am attaching a copy of Mr. ******'s Aspen Dental account ledger history for review.

With regard to any future treatment my prior response stands.

Sincerely,

Additional Notes

Complaint: Aspen dental advertises complete customer satisfaction or your money back. This has been a lengthy process that has cost me 2200.00 but the problem isn't fixed. I went in in the Spring of 2012 to get an artificial tooth/ bridge fixed. They first started me out with a marilyn bridge. It failed multiple multiple times. I finally asked them about changing the plan to fix my front tooth since the marilyn bridge isn't working. So they decided to later that year place a different bridge that required making two of my normal healthy permanent teeth into crowns and attaching the bridge with the artificial tooth all one solid piece. That solid guaranteed to work bridge has failed and it has been less than two years. I recently went back to Aspen Dental to have them check why my bridge was loose. They took xrays and couldn't explain why my bridge was loose. I asked well now what? They said that the only thing to do was to start over. I asked well is that on your dime or mine? The dentist looked at me and said it would be at my cost. I said no it shouldn't be because you guarantee your work and that I have had several issues with this whole process. I left pissed off and frustrated. They didn't charge my anything for the recheck and I asked why. The blonde/brown receptionist said nope you don't owe on it cuz we did the work. But yet they won't fix the issue unless I pay for it. I recently went to the dentist that my daughter and husband go to. I explained the issue with Aspen Dental and my bridge. He did an extensive oral exam with full mouth radiographs. He then went over everything with me. He told me that in his professional opinion, he would have not have done what Aspen did. He said that the roots of the incisors were/are way to short to support that heavy of a bridge and that is why my bridge has failed. He said they should have either made a bridge that involved more teeth so the bridge would have been more stable or have an implant done. He said that I should have never been left with what they did. I am so upset because this is my mouth that isn't fixed and now I will always have two crowns to maintain. I know have to go through the damn process again but this time with an implant to the tune of over 5,000.00. I have already spent 2200.00 at Aspen Dental that was guaranteed to work but yet it has not!!!!!

Desired Settlement: I want the 2200.00 I spent to be refunded back onto the care credit card I used. If not cash ir check would be nice! !!!!!

Additional Notes

Complaint: I went to Aspen Dental for an evaluation on March 28, 2014; at that time the dentist recommended several procedures. I received a treatment plan summary stating that my portion after insurance deductions was $960. I went back on August 8, 2014 to begin treatment. When I was called to the back I had to prepay for services before I was treated. When I sat down for treatment the dental assistant gave me papers to sign, one of which was an authorization for an “oral cancer screening”. I asked the dental assistant if the insurance company had approved the treatment which was $70, she checked and they had not, therefore I declined the screening. I realized at that time that I had not looked over the treatment plan summary closely enough; I assumed that it would only include the procedures the Dentist had recommended and nothing more. After I received services I looked over the treatment plan summary in detail and noticed a $70 Oral Cancer Screen and a $129 Oral B Power Brush over and above the services that the Dentist had recommended. I did receive the toothbrush; however I had declined the cancer screening. I feel these products / services should have been discussed before being added to the summary, it gives the impression of being dishonest and unscrupulous. When I received the explanation of benefits (EOB) from the insurance company, it stated that there were $600 in charges that were billed directly to me that were actually covered by insurance; however they were not submitted properly by Aspen Dental. The EOB stated that the services needed to be resubmitted with “dates of active perio treatment, dated xrays, dated perio charting, and narratives together for continuing necessity”. I spoke with Brittany at Aspen Dental as to why I was charged more than the EOB stated I was supposed to pay; she reviewed the account and said that I would need to speak with *******; she left a message for him to call me to discuss the charges. After not hearing back from ******* for 2 days, I called again on September 12, 2014 and was told that ******* was unavailable. I explained that I felt like ******* was avoiding my call and that if I did not hear from him by Monday I would contact the Attorney General’s Office. When ******* returned my call he was unpleasant. I explained to him that the EOB stated that Aspen Dental needed to resubmit the claim and that I should not have been charged for the services. He disagreed and said it was my financial responsibility even though they did not submit the proper paperwork. He stated that I needed to contact the insurance company and discuss it with them. After speaking with *******, I called the insurance company and found out they pay 90% of the charges in question and that Aspen Dental needed to resubmit the claim as stated on the EOB. I am confused as to why I was charged these fees in the first place; Aspen Dental should have adjusted my coverage properly, and charged me correctly. I never should have been charged full price for these services when Aspen Dental had the correct information for proper billing. Again, this appears dishonest and unscrupulous. I called ******* and explained the insurance company’s response. I also asked for a refund back to my credit card for the $70 cancer screening I did not receive, he stated that I had a $47 credit. I asked for him to (1) refund $47 back to my credit card; (2) resubmit the claim with the proper paperwork; (3) refund the portion of the claim covered by the insurance company once it is resubmitted, and he agreed. After the conversation with *******, I picked up a copy of my Account Detail from Aspen Dental to figure out the discrepancy on my account and the actual credit amount owed. After comparing the account charges to the treatment plan summary that I signed, I noticed yet another discrepancy in billing. The treatment summary that I signed stated that the charge for procedure code 4341, Perio SRP, 4 + teeth (UR), was supposed to be billed at $14.50 after insurance deductions. The account detail states that I was charged $59.50 for this service, a difference of $45. According to my calculations, I am due a refund in the amount of $632 for the following: (1) $47 current credit on my account that ******* agreed to process during our phone conversation on September 12, 2014. (2) The insurance company pays 90% of the charges that need to be submitted with “dates of active perio treatment, dated xrays, dated perio charting, and narratives together for continuing necessity”. Therefore I am entitled to a refund of the amount they cover ($540). (3) $45 overpayment of services for procedure code 4341, Perio SRP, 4 + teeth, UR. As of September 29, 2014, ******* has not refunded my credit card or resubmitted the claim with the proper paperwork to my insurance company. I am completely disappointed in the office manager, his dishonest and unscrupulous billing practices, and his negligence in resubmitting this claim to my insurance company.

Desired Settlement: Refund for the amount of charges that were overpaid / incorrectly billed. $632.

Complaint: I made an appointment for a dental hygiene appointment and was charged $55 upon arrival for said service. I was taken to dental chair where a dentist introduced himself and told me he was going to do a dental exam and that there was no hygienist in the building. then I was advised that I needed x-rays since it is policy that exam and exrays are due annually. I was further told by staff that it was "policy" to not advise scheduled hygiene patients when hygienists were to be absent in an effort to avoid losing business. Of course, I had additional fees due for services rendered in addition to the $55 I paid before services were rendered. My complaint is that this "policy" is highly unethical and unnecessary as I had no problem with getting annual exam and x-rays, and could have easily gotten them at the time of a real hygienist appointment without being tricked into two appointments per some alledged policy.

Desired Settlement: I desire a determination if this is indeed Aspen Dental policy as it is obviously practice, and to have it submitted to the AMDA Ethics Board for a review of practices, if not fraud, especially since I was charged in advance for services never rendered.

Complaint: I went to the Aspen Dental in Lebanon Tn back in 2013, Aspen Dental in Lebanon Tn billed my dental insurance for a perm partial, my dental insurance paid on that claim back in 2013. My partial that I received was a temp partial per **** the office manager, at the Lebanon store, after the second time my "temp partial" broke, I was told I was going to have to buy a perm partial, my insurance told me that Aspen Dental billed them for a perm partial. After, getting my insurance on the phone in the office with ****, back in June 2014, **** admitted that I got what the Dr had in my chart, and that is what I got. **** informed me that he would write off $900, for my perm partial. I am now getting a bill for $900.00. In this case I believe that Aspen Dental is committing fraud.

Desired Settlement: I want Aspen Dental to write off the balance of my account. I owe Aspen Dental $0.00

Business Response:

Dear *******,:

Thank you for sharing your concern about the balance due on your account.  I certainly do apologize for your experience at the Lebanon, TN Aspen Dental office.

I have received and reviewed your complaint filed with the Better Business Bureau.  Your original treatment quote did not include a FlexiCombo partial denture.  You were only charged for an Acrylic partial upper denture, which is in some cases the permanent denture depending on what services the patient chooses.  A review of your account shows that Aspen Dental has already removed $302.40 in previous balances owed by you.  Your total out of pocket payment amount for all the services provided you to date is $301.10.  As a final courtesy, I have removed the $900.00 that you are being billed for the denture that you received on July 25, 2014.

Complaint: During my initial visit, I was told by the dentist that I would require the removal of two permanent teeth to install a permanent bridge. The cost would be about $4000 (after insurance) and I would have to pay 60% up front with the balance due within 12 days of the initial deposit. Not a problem. Then...During my cleaning, I was told by another dentist that installing a permanent bridge may not be possible. He expressed concern that it may be in my best benefit to have braces for a year. He advised me that we should take "study models" to ascertain the direction of my treatment plan. I was further told that the study models would cost nothing since they have an agreement with the lab that provides services. They took study models of my mouth and I was assured that I would be called when more information was available about my treatment plan.Later I received a call from ******** letting me know that they need a "$2000 deposit" to send the "study models" out to the lab.Okay, now I am a little skeptical. But ******** calls it a mistake... "As I stated' (she said this no short of 9 times in the 5 minute conversation) "there is a cost for the lab".The 2 root canals that I need will also be part of that $2k deposit. So basically, all of my treatment is halted until I give them $2000. This is regardless of the determination of the need for braces for one year OR if they're going to recommend the extractions and bridge..... Because nobody knows what my treatment plan is until these "study models" are sent to the lab. I can understand that if there truly is a cost for the "lab", that I should bear financial responsibility for the lab costs and consultation. But, $2k as a down payment for work that may not be needed is a bit steep. Promising a 'refund' if I am referred out is not a resolution. The fact that they stopped all dental treatment is ridiculous. There is no reason I should not be allowed to pay the co-pay on other work and/or lab fees to determine the best treatment plan.

Desired Settlement: DesiredSettlementID: Other (requires explanation) I would like someone (other than Samantha) to contact me and discuss continuing my treatment plan. It is completely insane to require me to pay a $2,000 deposit for work that is only a $236 co-pay (root canals). I cannot image that the remaining $1,764 is the cost of evaluating "study molds" to determine a treatment plan.

Complaint: I went in with extreme tooth pain in my lower left jaw. I had the molar extracted and during the procedure Dr ***** cracked my wisdom tooth as well. I was then charged for 2 teeth extractions and it took over 2 hours for the dentist to get out both teeth, during which time he took a break and left me in the chair with my mouth open. I went home in worse pain than when I arrived and with very little pain medication. Four days later I began to develop signs of dry socket so I went back to see Dr *****. I was in so much pain I couldnt sleep, eat or function normally. A fellow dentist was helping out for the holidays and informed me that I had dry socket after only the briefest look in my mouth. She packed my socket with paste and when I asked about pain medication I was treated like an addict and told that they did not prescribe pain medication for dry socket, that it would not help it. That is completely ludicrous as pain medication is the ONLY thing that was giving me any kind of relief. I told her to please send in Dr *****. I could not believe she was going to send me home in such extreme pain with no medication. Dr ***** came in and gave me more medicine although he said it was not their practice. The next morning I woke up to even worse pain and instead of dealing with Aspen again I went to the emergency room. The doctor there said I did NOT have dry socket but I did have one of the worse infections hes seen in anyone's mouth. He prescribed strong antibiotics which Aspen never did and I also have to go back to the emergency room 3 more days to receive antibiotic shots as well. The infection could have cost me my life, it really was that serious and I feel like the staff at Aspen were very negligent.

Desired Settlement: I would like a refund of all monies I have paid as well as my emergency room services paid as this was completely their fault for misdiagnosis and negligent treatment.

Dear Mr. ********:

Thank you for sharing your concerns about the treatment that you
received.  I certainly do apologize for your experience at the Carrollton, GA Aspen Dental.
I have reviewed your complaint statement and Aspen Dental account with the Office
Manager, ********, the dentist Dr. Sales and the Regional Manager, ****.  With regard to the extraction of the second tooth
(# 17), I have been advised that it was done at your request.  In fact, you paid the additional fee required
for that service on that date.  Following
the extractions on December 27, 2013, you were prescribed Ibuprofen 600 mgs (pain)
and Clindamycin 150 mgs (antibiotic).

Given your noted dissatisfaction and as a courtesy, I have refunded the total $94.80 paid by you
via your Visa card on December 27, 2013.
Please allow 3-5 business days for the $94.80 credit to post on your
Visa card account.  Any consideration for
reimbursement of payments made by you to other providers will require
supporting documentation.  Upon receipt
of that documentation a clinical review will be done, and a reply will be sent
to you.
I appreciate you giving me the opportunity to respond your inquiry.  I am available for any further questions at
the below contact information.

Sincerely,

Consumer Response: Better Business Bureau:

I have reviewed the response made by the business in reference to complaint ID *******, and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

I do appreciate the swift response however there was pertinent details that were not documented or addressed correctly. First, I did not request my wisdom tooth be extracted in my initial visit. In the 2 hour process of removing my molar Dr. ***** cracked my wisdom tooth leaving the nerve exposed. I felt that being it was his error he should not have charged me period and was unpleasantly surprised when I got to the checkout area and was told how much my bill was. I felt I had no other choice but to pay what was asked as I did not want to chance an adverse credit reporting for the account. That being said I appreciate you refunding my $94.00 as I feel that is only right. On December 31, 2013 my insurance company issued check number ********* in the amount of $226.50. I would like that refunded as I feel that is only right as well and included in my original request of receiving all monies back. As far as supporting documents from my subsequent emergency room visits, I will need an email address to send said documents to and will be more than happy to do so. Currently, I am still under the care of a medical physician due to the horrible infection that occurred from having the teeth extracted and have since had further x rays to reveal a piece of the wisdom tooth that myself and my insurance company paid to have removed is in fact still in my jaw and compacted. The negligence of Dr ***** has cost me much pain, sorrow and money.

Regards,
****** ********

Complaint: First of all my wife went there and her bill to them was 50.00 a month. When I went I told them all we could afford was 75.00 a month.Totaling 125.00 a month for both of us they said fine no problem. Then we get a call from from credit card services for my bill saying we have to pay 125.00 a month on my bill which we could not afford and there interest rate was out of this world. So we could not pay my wife's bill until the medical card was paid.Which we have just paid off.Now my wife has a wisdom tooth coming in on top of her dentures which they were aware of but did nothing about it.Now we have to pay 625.00 to have 2 wisdom extracted for her.My wife called Aspen Dental to have my broken dentures repaired which are still under warranty and a new liner installed in both hers and mine they refused to give us this service because we have outstanding bill. They also informed me that I have a outstanding bill of 176.00. When everything was supposed to go on the Aspen dental credit card. I think they are a bunch of crooks and do not appreciate being treated in this manner

Desired Settlement: I think they need to replace my dentures or at least repair them they have never fit correctly and 1 tooth in front has broke off. They said the denture liner was part of the service.  Dear Mr. and Mrs. ********:

Thank you for sharing your concerns about your treatment and the billing on your accounts.  I certainly do apologize for your experience at the Florence, SC Aspen Dental.

I have reviewed your Better Business Bureau Complaint and both your Aspen Dental accounts.  I have also performed audits on both accounts.  With regard to Mr.
**** ********, your original treatment estimate shows a patient responsibility of $1,491.00.  To date you have paid $1,461.00.  To honor the original
estimate and as a courtesy we have removed the $174.88 balance showing due on your Aspen Dental Account.  Our records indicate that the initial minimum payment required via the Genesis Financing that you were approved for was $75.00 per month however; please note, that you agreed to the 12 Month Interest Free program and in order to take advantage of that offer you would have had to remit payments of $119.00 per month for 12 consecutive months (see enclosed documentation).  This would also have been reflected on your Genesis billing statements.

With regard to any treatment concerns, the Florence, SC Aspen Dental Office Manager, ******* has left you messages attempting to contact you to resolve any issues that you may be having with your dentures. I myself sent you an e-mail on January 10, 2014 and I spoke directly with Mrs. ******** on January 16, 2014

With regard to Mrs. ******* ********, she contracted for the 50/50 In House payment plan on April 19, 2012.  Per my audit of her account the remaining balance of $318.50 is correct.  The last $50.00 payment received on her account was on October 19, 2012.  As a courtesy I allowed Mrs. ******** to be seen at our Douglasville, GA Aspen Dental office on January 17, 2017.  In order to receive any further treatment, the balance in full must be paid.

Mr. **** ******** Mrs. ******* ********

Complaint: I had 2 partial plate dentures made at Aspen in Rutland, VT. I had to go back 2 times for adjustments. A few days after receiving them I departed for Florida for the winter. After getting to Florida and wearing these teeth for a few weeks I realized that I could feel my heart beating in my teeth. I called Aspen in Rutland VT and they said to go to an Aspen in Florida and they would take care of me. I went to see Aspen in Mt. Dora FL. on November 29,2013. They listened to my complaint and said they would have to consult with Rutland VT. They did and Rutland called me and said to make an appointment with Aspen in MT.Dora and they would make me a new plate.Mt. Dora said my teeth were being stressed because I needed a cap on the tooth that the plate hooked to. I made an appointment and got in to see them on Dec.16,2013. After full set of x-rays and an examination they told me they would have to do a crown on two top teeth and "maybe" a root canal depending on how they found the tooth when they got to drilling. They took me into the office managers office and she explained that the new charges would be approx. $3000. I told her that I could not afford that and she told me to call the office in Rutland an request my money back. I did this and they told me I could not get a refund. They would take the price of the plate off my bill when I had the work done. I again told them I could not afford this and I wanted my money back. I called Mt. Dora back and asked if they could help in any way. They said they would call Vermont. They did call VT. and when they called me back they said I have to come to the VT office for evaluation. I will not be back in Vermont until mid May.

Desired Settlement: I cannot wear this plate. It was made wrong according to Mt.Dora and therefore would like my money back. This is in the amount of $759.20

Business Response: Dear Ms. ******, Thank you for your e-mail concerning the issue with your partial denture.  I do apologize for your experience with the Rutland, VT Aspen Dental.   I have reviewed your complaint statement with both the Rutland, VT and the Mt. Dora FL Aspen Dental offices.  Given the circumstances, Aspen Dental (upon receipt of the returned partial upper denture) will refund the $759.20 payment amount to the ****** ***** ******account used to make the payment.  Please contact ****** at the Mt. Dora,FL Aspen Dental ###-###-#### to make arrangements to return the denture.  Upon receipt, the $759.20 refund will be processed back to****** *****.  Please allow 5-7 business days after returning the denture for the refund to be posted back to ****** *****. Thanks,  ********

Complaint: This company failed to provide/perform the agreed upon dental work in a timely fashion. I had 3 scheduled and met appointments for and an additional 3 appointments that where rescheduled by the Gainesville Aspen Dental office. My insurance carrier covers the dental work I requested but the business office manager stated that unless I pay $45 for a pretreatment, the dentist will refuse to continue the work. In addition to the required $45 fee, I was told that I needed a special rinse and tooth past that totaled $60 and a toothbrush that cost $130. When I politely declined these items, I was escorted to the front desk to reschedule my next appointment. At that point I asked if my insurance was billed for any service and the office assistant didn't know when in fact my insurance was billed for services not rendered.

Business Response:

I am in receipt of your complaint filed with the Better Business Bureau.  I do apologize for your experience at the Gainesville, FL Aspen Dental.  I have audited your account and see the only service charged to your insurance was for the cleaning done on November 06, 2013.

For your review I have attached a copy of your Aspen Dental account ledger showing all transactions.
Thank you for allowing me the opportunity to respond to your
inquiry.  Should you have any further questions, you may reach me at the
contact information noted below.

Business Response:

****,

Per the patient's dental chart notes from the Hygienist, Mr. ********* did receive a cleaning.  His insurance paid $10.00 and Aspen Dental absorbed the rest ($75.00).  The $45.00 he mentions was for Irrigation with Chlorhexidne, which was also provided to him. That charge was removed from his account as well.  There have been no out of pocket costs to Mr. ********* for his treatment.

Consumer Response: Better Business Bureau:

I have reviewed the response made by the business in reference to complaint ID *******, and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

Thank you for taking my call earlier today about complaint #*******.  According to the response from Aspen Dental, they in fact billed my insurance company because my chart stated the work was complete.  However, the work was not completed and this isn't a blatant mistake of mis-billing.  This is a clear case of insurance fraud in which my chart was altered to reflect the service being done.

In my original complaint, I stated that the dentist would refuse to see me if I didn't opt for the irrigation for $45.  When I arrived for the service, I spent approx. 8 min with the hygienist who focused on selling me product.  After I politely refused the product until I did further research on it, I was sent up front to schedule my next appointment. At this time I asked about the cleaning (the service my insurance was billed for) and the hygienist and the receptionist stated that they are done on different days.  At this time my next appointment was scheduled which was never met due to the un-organization of the office, its management and its dentist.

I have contacted Aspen Dental and validated my statements which I have proof that my charts were falsely documented and can prove that this said service was not completed.  I did contact Aspen Dental and a service representative stated that he personally called the Gainesville location and was told by the business office manager that my chart stated the work was complete.  This does coincide to the final response on my complaint.

My complaint was not for any out of pocket expense but rather a change in the company's practices and policies.  I have only made claims to my personal experiences and not what I witnessed in this Gainesville location with other patients.

I find trust in the BBB and have never questioned  the ratings of the participating business'.  I want to continue this trust and not feel as if the BBB is showing favoritism toward paying business'.  Please review my original complaint and take the proper action.

I am in contact with my insurance company as well as  the Coalition Against Insurance Fraud and the Florida Office of Insurance Regulation.

Sincerely,
*** *********

Business Response:

I have reviewed the patient’s complaint and claim that he did not receive a cleaning on November 06, 2013.  Per the chart notes input by the Hygienist on that date, the service was provided.  As a courtesy to the patient and in response to his dissatisfaction, I am removing the charge and refunding his insurance company for the $10.00 payment they made.  Please add this information to Mr. *********’s file.

Additional Notes

Complaint: I would like to notify you of the unbelievably horrible experience and treatment I received from Aspen Dental. I went through the Aspen Dental location in Enfield CT to get my wisdom teeth removed. At my first appointment my X-rays were taken and an appointment was set for me to get my teeth extracted on December 13th at the East Longmeadow, MA location. The only reason I agreed to go through the East Longmeadow, MA location was due to the fact that I was told that there was no available appointments at a closer location any sooner than that. At that first appointment I was quoted an amount of $360.50 out of pocket which I financed through my Care Credit card. I signed all of the appropriate paperwork signing off on this amount. Three days before my scheduled surgery I received a call from ****, at your Enfield location, whom I had already dealt with. She left me a voicemail and requested that I call her back to go over a few things. When I called her back she notified me that I had a remaining balance due of around $400, when I asked her to explain to me why I owed this specific amount she tried to overlook it like it was no big deal. She said, “We’re currently having issues with our computers and the estimate I previously gave you was wrong, it must have something to do with your insurance.” When I asked her to further explain this to me she said “It’s complicated to explain, and we’ll try and go over it with you when you come in for your surgery and you may or may not be billed after your surgery.” When I became frustrated she suggested that I cancel my appointment, which I was very tempted to do and would have if my teeth weren’t already causing me such discomfort. When I arrived for my surgery I was notified that I owed an additional $356.50 and had no other choice but to put this amount on my Care Credit card in order to have the surgery. After my surgery, I was not accompanied by a staff member out of the chair after I had been knocked out by an intravenous. I had two people attend the appointment with me and neither one of them was notified that I was all set and ready to leave. I was told to walk down a hallway to meet the people that were there to take care of me post-surgery. I then fell in the hallway with no supervision. The staff let me lie in the middle of the hallway where my mother had to literally pick me up off of the floor. My surgery was done on a Friday, I understand that office hours are not offered on Saturday’s but I had issues when I woke up on Saturday morning (the day after my surgery.) I was given both an emergency number as well as my doctors “supposed” cell phone number. I called the emergency line Saturday morning at around 9 a.m. I was told that they would page my doctor and I should be hearing back from him within twenty minutes. When I had not heard back from anyone 4 hours later I called the cell phone number where it immediately went to voicemail. I then called the emergency line back explained I had made a previous call. The emergency line then called me back three times over the next hour asking if I had received a call back from my doctor and when I stated no they said “okay we’ll try paging them again.” The phone call I received back was not from my doctor but from the financial aid woman ****. I had specific questions about my stitches. I felt that they did not cut down my stitches short enough because they were digging into the sides of my cheeks causing me to be in a high level of pain and made the swelling of my cheeks even worse. When I described this to **** she then put me on hold, when she came back on the line she suggested that I take a pair of scissors and cut them down myself or try to put numbing cream on my cheeks. I felt that the suggestion of taking a pair of scissors and snipping them myself was the most unprofessional piece of advice she could have ever offered me. Not only could I not even open my mouth wide enough to even attempt to do this but that can cause serious infection, as well as inflicting further pain amongst my wounds. My experience with Aspen Dental has been absolutely horrific and unprofessional from beginning to end in all aspects. I’ve had issues with them on both the medical and financial side of things and will tell everyone I come in contact with looking for a dentist to go anywhere but Aspen Dental.

Desired Settlement: I do not feel that I should be responsible to pay the remaining balance that I was required to pay on the day of my surgery, I had already signed all of the paperwork showing that I was only responsible for the $360.50. I feel that I should be reimbursed in the amount of $356.50 because it was not initially disclosed to me and it seemed to have been an error on Aspen Dental’s behalf. I will be filing a complaint with the Connecticut Department of Public Health, the Massachusetts Department of Public Health, the Better Business Buarea as well as the Attorney General’s office.

Business Response: Dear Ms. ********: Thank you for sharing your concerns about your treatment, and the insurance billing on youraccount.  I certainly do apologize for your experience at the Enfield, CT and East Long Meadow, CT Aspen Dental offices.  I have reviewed your Better Business Bureau complaint statement and performed an audit on your Aspen Dental account.  On January 03, 2014 Aspen Dental received payment from your dental insurance carrier ******** Life.  Per the explanation of benefits provided by ********, your responsibility for the services done on December 13, 2013 was$335.80.  You paid a total of $717.00($360.50 on December 03, 2013 and another $356.50 on December 13, 2013).  The amount of the overpayment was $381.20,which is being refunded to your CareCredit account used to make payment.  Please allow approximately 5 business daysfor the credit to post back on your CareCredit account. I am very sorry for the miscalculation of the estimate of your out of pocket costs. Please accept the enclosed $50.00 MasterCard gift card as a token of my apology.  This is not meant to serve as a solution to the problem, but to show you that your satisfaction is important tome.  A copy of this correspondence has been sent to the Better Business Bureau in response to your complaint. I appreciate you giving me the opportunity to resolve this to your satisfaction.  I am available for any further questions at the below contact information. Sincerely, Ron Hobbs

Additional Notes

Complaint: I went into Aspen Dental due to a cracked tooth. They suddenly started me on a Care Credit Credit Card which charged me OVER $5,000.00. I now have that to pay off and interest that is up to $70/month! Just knowing that I know I will NEVER be able to pay this off. They now have given me root canals but have yet to place a crown on them. I am a fragile patient and one nurse in there who is blonde with an accent has always been very rough and rude to me, telling me to calm down, 'I don't know what your problem is'. One of the nurses in there who is African American who wants to cut my gum and G-d knows what else she wants to do has always been very unpleasant to me also. I am not sure why I have this HUGE bill and why I was charged so much up front when my crowns STILL are not on. I am scared to go back to these people on Main Ave in Norwalk. I am unsure of this company and now have this HUGE credit card that I have to pay off that will likely, take me years to pay off. PLEASE HELP ME!!

Desired Settlement: normal crowns are $1, 200 each. So, okay, if I have to have two, then I should ONLY have a credit card bill of $2, 800? Am I right?

Business Response: Thank you for sharing your concerns about your treatment and
financing.  I certainly do apologize for
your experience at the Norwalk, CT Aspen Dental.

I have reviewed your Better Business Bureau Complain Statement with the Norwalk,
CT Aspen Dental Office Manager, ********.
I have also performed an audit of your Aspen Dental account.

In response to your inquiry about the cost of your treatment, I am enclosing with
this letter copies of your Financial Acceptance Form, your original treatment
estimate, and your Aspen Dental account ledger showing all transactions on the
account.  Your total treatment has
consisted of much more than two crowns that you have yet to receive.  As part of your overall treatment plan you
have had root canals, fillings and periodontal therapy done as well.  Please contact ******** at the Norwalk office
to discuss any questions about your treatment and she can have the dentist
discuss it with you.  ******** has also
advised me that you have decided to switch to the Fairfield, CT Aspen Dental
office to complete your treatment.

I appreciate you giving me the opportunity to resolve this to your
satisfaction.  I am available for any

Additional Notes

Complaint: I went to this company primarily because of the weekend hours of operation to have my tooth examine that has been bothering me lately. I was informed I needed a root canal but explained to them I am unable to cover the out of pocket expenses this procedure involved and ultimately went with the removal of the tooth since this procedure is covered by my insurance. Besides this procedure I received a pricing pdf for other issues that I was having which included pricing at $0.00 for cavities to be filled. I went ahead and scheduled these since obviously there was no out of pocket on my end and I had informed them earlier about my budget. I scheduled and had the fillings placed and later received a 400.00 bill after the original quote said $0.00. I contacted ****** (office manager) after speaking with my insurance which said they placed the white resin instead of the silver which is covered by my insurance and she basically gave me the run around and I am now obligated to pay this balance which seems a bit criminal to me since I entrusted their quote and I could of had this whole procedure covered in full. Than to top it all off ****** says it is just like going to the hospital if you had an emergency … I do not know what type of Medical insurance she has but it is not like going to the emergency room.. I have a 100.00 co-pay if I went to the emergency room not 400.00 Please help me, **** ****

Desired Settlement: I would like the entire amount adjusted I was quoted 0.00 and I should pay 0.00 - I have the pdf showing the quote and an honorable business would stand by what they quoated and not put in a filling type that was not covered by my insurance and instead use what is covered by my insurance and expect me to pay the additional amount.

Business Response:

Thank you for sharing your concern about the billing and balance remaining due on your account.  I certainly do apologize for your experience
at the Holland, OH Aspen Dental.

I have reviewed your Better Business
Bureau complaint statement and I have performed an audit on your Aspen Dental
account.  The reason your account is
showing a balance due of $408.00, is because your insurance company ******
********* paid less than what was estimated for the fillings done on September
14, 2013.  Also, as part of that payment
****** ********* did not require that Aspen Dental take the provider discounts
that were included on your original estimate.
The difference in payment amount was $124.00.  The amount of provider discounts not taken
based on ****** *********’s response is $284.00.  These two amounts total the $408.00
balance.

Given the circumstances, Aspen Dental
has decided to honor our estimate and have applied the $284.00 provider
discount.  Also, as a courtesy and an apology
for any inconvenience to you, I have removed the $124.00 amount resulting from
the insurance payment.  The account now
reflects a zero balance due. I have included an account ledger history with
this letter showing the adjusted balance.

I appreciate you giving me the

Complaint: This is the second time I am sending this complaint as I did not hear back from the BBB. I may not have recognized your email and deleted it as junk mail, but I'll try again. I went to Aspen Dental for the new patient free check up and cleaning. At this appointment I was told that I had periodontal disease and that if I didn't have treatment to take care of it my teeth would fall out. I told Aspen Dental that I didn't have dental insurance and that I couldn't afford the treatment. I was told that I HAD to have the treatment and they'd sign me up on a payment plan with Care Credit. I did sign up because I believed what they were telling me and trusted that they wouldn't talk me into the $1854.00 treatment if I didn't need it. So far I've paid about half the charge , but now I'm paying interest on it too. I did think I was doing what I needed to do until I talked to others who could not believe how much I was charged. The dentist I had gone to when I was on my family's plan was appalled and I want to go back to him; a reputable dentist that I can trust. I now have a balance of $1098.19 that I owe Care Credit and a zero balance with Aspen Dental because Care Credit has paid Aspen Dental in full. I called Aspen Dental and told them that I would not be continuing treatment with them. They were very rude ,but don't care if I come back because they were paid up front for treatment that they hadn't even done. I want Aspen Dental to refund my balance to Care Credit so that I can get out of this nightmare without my credit being ruined.

Desired Settlement: I would like Aspen Dental to refund the balance owed to Care Credit. I want out of this scam without my credit being ruined.

Business Response: We are in receipt of a Better Business
Bureau complaint filed on your behalf by ***** *****.  As you are the adult patient of Aspen Dental,
I am responding directly to you.

I have reviewed the complaint statement.  I have also reviewed your prior contact in
July 2012 with ******* of the Aspen Dental Patient Satisfaction
Department.

On July 02, 2012 you spoke with ******* and
stated that you were satisfied with Aspen Dental.  You requested that the $450.00 credit on the
account at that time be refunded, and from then on you would pay as you
go.  A $450.00 CareCredit refund was
submitted by ******* and processed on that same day. Your last visit to the
office was on November 28, 2012 for follow-up hygiene care, for which you paid
$204.00 via your CareCredit account.

If there is an issue or concern that you
wish to discuss regarding the treatment provided you at Aspen Dental or your
Aspen Dental treatment or the account, please do not hesitate to contact me
directly and I will be happy to speak with you.

I appreciate you giving me the
opportunity to respond to this inquiry.
I am available for any further questions at the below contact
information.

Better Business Bureau:

I have reviewed the response made by the business in reference to complaint ID 9729330, and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

Aspen Dental said that they refunded my Care Credit account $647.00, I
have not seen any verifcation of that in any of my billing statements.
I did receive a letter that they had refunded the price of a defective
toothbrush, $197.00. I do not want to accept the refund until I know
they really did refund my account $844.00 of the $2058.00 leaving me a
balance of $334.00. Please advise me of how to get this in writing
from Aspen Dental.

Regards,

************** *****

Business Response:

I want to be as clear as possible in this final response to Mr. *****'s complaint.  On July 12, 2012 Aspen Dental processed a $450.00 refund back to CareCredit.  On September 24, 2013 as a courtesy an additional $197.00 was refunded.  The total amount refunded to CareCredit is $647.00.  Mr. ***** can confirm the refund amounts by contacting Care Credit directly at ###-###-####.  Any remaining balance due with CareCredit must be resolved with them.  Please see the account ledger which reflects the charges and the refund amounts.

Sincerely,

Additional Notes

Complaint: A while back, I took my kids for dental cleanings. My son was told he had a tooth that would need oral surgery to bring the tooth down. Told me to get his braces on and then the surgery. But, he needed a partial until the braces were put on. I purchased the partial. It was $500. I had to finance the purchase the of the partial. We went back to pick up the partial. They said the he would use the partial until the braces were put on....he would only wear it for approximately two weeks. TWO WEEKS! I couldn't believe I was advised to buy this partial for only two weeks! I had to pay this off first before I could put braces on him. After quite a while later, I finally got dental insurance. The closest dentist my insurance would cover was Aspen Dental. I wasn't happy, but it is all the choices I had. Took my kids and myself for x-rays and cleanings. I was given a quote of like $450 for a cleaning and products. When I arrived, I talked to the lady about not needing the electric tooth brush and things like that. I couldn't afford to pay that. She said she needed to redo my estimate because it was billed wrong. I thought, oh good....it will be better. She took off the toothbrush and products, and added what she needed to. Suddenly, my bill became over $900!!!! I couldn't believe it! For a cleaning! I have never had problems with my teeth...not even a tooth ache! I told her that I couldn't pay that. She suggested their credit program. Then she said she could try to combine some things to reduce the bill. After everything, it was still over $700! I said forget it. As far as my son, I was told my son would need the two part cleaning. After insurance it would be around $90 for the cleanings and the toothpaste and mouthwash. I agreed. He got the first cleaning, then scheduled the next one which was included. Took him in today, and they said it would be $85 extra. I am fed up with this place! I asked for my x-rays so I can take them to someone else.

Desired Settlement: I would like a refund of the partial that I purchased that I was lead to believe that he needed. Not for just two weeks! I would like to see a policy change of how they change the cost of procedures and services.

Business Response:

Thank you for sharing your concerns about your account.  I certainly do apologize for your experience
at the Troy, OH Aspen Dental.

I have reviewed your complaint statement with the Troy, OH Aspen Dental Office
Manager, ****** and the treating dentist and practice owner Dr. ****.  The charge for the partial denture that you
are requesting a refund for is more than three years old.  Unfortunately, no refund will be given
towards that service.

I  appreciate you giving me the opportunity to respond to your inquiry.  I am available for any further questions at
the below contact information.

Additional Notes

Complaint: Aspen Dental is attempting to collect $779.85 using 2 collection agencies. I disputed these charges to Aspen Dental in a letter dated 4/04/13. No response was received. I then received a collection notice from ******** Credit, Inc dated 8/08/13 which I responded to on 8/16/13. No response from them either. I've now received a collection notice from ********* ******** Services for the same disputed billing. Copies of my afore-mentioned letters and of the quoted treatment plans are available if needed.

Desired Settlement: Correct billing to quoted price.

Business Response: Thank you for sharing your concerns about the balance due on your
account.  I certainly do apologize for
your experience at the Lincoln, NE Aspen Dental.

I have reviewed your complaint statement and performed an Audit on your Aspen
Dental account.  The balance on your
account is a result of changes in your treatment from the originally quoted
estimate.  Also, Aspen Dental did refund
$522.90 to ********** in February 2013.
Given the circumstances and as a further courtesy to you, I have removed
the $779.85 past due balance.  Your Aspen
Dental account now reflects a zero balance due.
The collection agency is being notified to close their account.  Please note that no information concerning
this account has been reported to any credit bureau or credit reporting
agency.  For your review I have included
a copy of the account ledger history showing the adjustment and the zero
balance.

I appreciate you giving me the opportunity to resolve this to your
satisfaction.  I am available for any
further questions at the below contact information.

Additional Notes

Complaint: I had a set plan of dental work to be preformed. Aspen told me what my out of pocket expenses would be after insurance paid their part. However, the last filling I had on my front tooth, the insurance did not cover. Aspen never pre authorized this service, therefore they and me were unaware that the insurance would not cover this filling. After 2 weeks of having the filling, I returned to aspen for another service that involved an xray. As the xray was being read, I asked about the filling, and how it was still hurting. The Dentist came back and told me there was an air bubble under neither the filling, and he would like to fix it free of charge that day. I allowed him to do this. two weeks after fixing it, I was in the bathroom brushing my teeth, and had a sharp pain in the front of my mouth. When I looked in the mirror, I noticed the filling was almost completely gone. Like it had fallen out. I have contacted the officer where service was done about 12 times since then. They refuse to repair it. And they refuse to reduce the bill. I refuse to pay for a substandard service that involves my health and my teeth. They have sent this bill to a collection agency that is reporting to my credit.

Desired Settlement: I offered the collection agency 100.00 to pay for the products used, but refuse to pay for the dentists time, because the service was a failure. The collection agency agreed, but will not put it in writing. I want aspen to mail me an agreement stating they will accept a one time payment in the amount of 100.00, remove negative marks from my credit scores (all 3), stop the collection agency and show the bill "paid in full".

Business Response:

Thank you for sharing your concern about the balance showing due on
your account.  I certainly do apologize
for your experience at the Lansing, MI Aspen Dental.

I have reviewed your complaint statement and audited the account.  The $284.00 amount showing delinquent is a
result of non-payment for services by your dental insurance company for the
date of service April 26, 2010.  Per your
insurance carrier, your total responsibility for that date of service was $309.00.  You did pay $25.00 via your Visa card on that
date, hence the $284.00 remaining due.  For
your review I am including with this letter copies of the Explanations of
Benefits provided by Guardian Life Insurance to Aspen Dental.

Per our telephone conversation of today and as a courtesy to you, I have adjusted
$206.00 off the balance due.  Your new
balance is $72.00.  Upon receipt of
payment I will contact the collection agency with instructions to close the
account and to remove any information reported concerning this account from the
credit bureaus.

I appreciate you giving me the opportunity to resolve this to your
satisfaction.  I am available for any
further questions at the below contact information.
