I contacted the Health Insurance Marketplace on or about December 2, 2015 in response to a letter regarding the Open Enrollment Period November 1, 2015 through January 31, 2016. I was transferred to the representative, Jovanny **** who took my personal and financial information. I informed Jovanny that I currently had major medical insurance through BlueCross BlueShield until December 31, 2015 and I needed to choose a new major medical plan to be effective January 1, 2016; that BlueCross BlueShield notified me by letter that my PPO Plan would be discontinued on December 31, 2015. Jovanny enrolled me in a plan from National Health Insurance Company with a monthly premium of $108.99. I reiterated to Jovanny that I wanted major medical insurance comparable to coverage I had previously with BlueCross BlueShield.
On December 9, 2015 a charge of $108.99 was deducted from my checking account, however, I had not received any documentation or package to inform me of my coverage. I called several times but was placed on hold for an extended amount of time until I had to hang up. I called again and left a voice message. On or about December 17, 2015, I received two (2) ID cards from Life Styles Innovations and NHIC National Health Insurance Company along with a brochure from L.I.F.E. Association. After receiving these cards, I called again and left a message inquiring as to why I had not received my insurance policy and a package outlining my medical coverage. I was told by a representative that they have been backed up because so many people were enrolling at this time. Finally, at the beginning of January 2016, Jovanny **** called back and left a voice message saying that he received my message and to call him back.
I called the Health Insurance Marketplace again and was informed that I had no major medical insurance policy at this time, but an application was put in late in December and I would not be covered until February 1, 2016 - that I must still choose an Insurance Company that is recognized by the Marketplace. I informed them of the two (2) ID cards I received. They informed me that these cards are for TRIO MED and are not for major medical insurance coverage.
On January 12, 2016, a second premium of $108.99 was deducted from my account. I called your company again to complain and cancel this inappropriate insurance that Jovanny **** enrolled me in and requested a full refund due to the fact that I was misled to believe that this was a major medical policy. I was informed that this issue would go before a Review Board.
On January 16, 2016, I received a call from Jovanny **** regarding this issue. I reiterated that on our very first conversation, I requested coverage for major medical insurance - that I was not interested in any medical supplement or accident insurance. He gave me an email address to send a cancellation request in writing to cancel this insurance.
I, ****** ******, am canceling this membership and/or insurance plan coverage in writing and requesting a full refund of two (2) premiums of $108.99 each, totaling $217.98.
