*************************** of The Center for Ocular Prosthetics, LLC has submitted a fraudulent claim with Community Health Plan of the **************** for an uncompleted ocular prosthesis. She has cancelled the appointment for the adjustment that was needed and refused to provide copies of the patient records, which is against policy and a basic professional code of conduct. I came from ******* on 5/20/21 and had an ocular prosthesis made on 5/21/21. It needed several adjustments, which is typical for a prosthesis. When I returned to ******* I had the eye and the prosthesis examined, the doctors noted the adjustments that were needed.The prosthesis was hurting and was very uncomfortable to wear. It was creating painful abrasions, inflammation and even cysts. I came for an adjustment on 7/28/21, to reduce the iris and adjust the limbus, however the prosthesis was adjusted in the posterior which made it too thin and sink into the eye socket. There was no eye mobility with the prosthesis and its hurting even more. ********* acknowledged the prosthesis didn't fit well at all and scheduled a follow up for the following Friday 8/6/21. However, on 8/5/21, the day prior to the appointment, she texted my phone and cancelled the necessary adjustment. Since I am unable to wear the prosthesis as is, she also stated in the text message, that she would not charge for the prosthesis. (I can provide a copy.)Furthermore, she refused to answer any of my calls and messages and avoided any responsibility for a prosthesis she made, which has damaged my eye. She submitted a false insurance claim on 8/4/21, the day before she texted me to cancel, which was paid on 8/31/21. Since the money has been paid for a prosthesis she didn't delivered, she committed state insurance fraud, which is now being investigated by **************** ***************** ************* is the worst ocularist I ever had encountered and should not be operating in the medical field. She has demonstrated, she will do anything, including lie, gaslight and steal to stay in business and has absolutely no concern for delivering a suitable prosthesis, to disabled patients, who depend on this durable medical equipment in their every day lives. She was well aware the eye was hurting, but she cancelled the needed adjustment, the day after she submitted the claim on 8/4/21. She is a dishonest unprofessional, who hides behind the historical reputation of Mr. *********************** who started the Center and is a respected member of the Ocularist Association and a pioneer in the field. Its unfortunate that ************* has been put in a position to destroy his legacy.
