Programs

Visiting Nurse & Hospice Home is a community-based, non-profit provider of hospice and palliative care, serving thirteen counties in Northeast Indiana. Our hospice program provides compassionate care for the terminally-ill and support and assistance to the family via a team of professionals including: medical director, nurses, home health aides, social workers, non-denominational chaplains, volunteers and bereavement counselors. Visiting Nurse & Hospice Home provides care in the patient's home, in a nursing home or in the agency's 14 bed in-patient unit, Hospice Home, the area's only facility dedicated to care for the terminally-ill. Hospice care is generally paid for by Medicare, Medicaid and private insurance. Patients without resources or insurance may qualify for the agency's charity/sliding fee program. The majority of the patients cared for by Visiting Nurse & Hospice Home are elderly but t he agency serves patients of all ages. Hospice care is not limited just to cancer patients but also patients diagnosed with other life-limiting conditions such as Alzheimer's, end-stage cardiac and kidney, ALS, Parkinson's and others.
