I HAVE CALLED DENTAL WORKS GREAT LAKE AT XXX-XXX-XXXX TO HAVE THEM BILL SEVERAL BILLS TO OUR ***** INSURANCE. THEY STILL HAVE NOT BILLED SEVERAL DATES
MY DAUGHTER WENT FOR DENTAL TREATMENT IN APRIL AND SHE PROVIDED HER ***** CARD. RATHER THAN BILL THE INSURANCE SHE PROVIDED THEY WENT ON AN OLD ACCOUNT OF HER BROTHERS AND USED HIS INSURANCE, SHE IS NOT COVERED BY. SO OF COURSE HER 4 DATES OF TREATMENT DENIED. I CALLED AFTER I GOT A BILL THAT SAID HER INSURANCE WAS NOT VALID AND THEY SAID THAT THEY BILLED SOMEONE ELSE'S INSURANCE. I VERIFY THE CORRECT INSURANCE INFORMATION AND ASK THEM TO REBILL ALL 4 BILLS TO THE CORRECT INSURANCE. AFTER SEVERAL MORE CALLS FOLLOWING MONTHLY BILLS ONLY ONE DAY IS BILLED TO *****. I RUN A MEDICAL BILLING COMPANY FOR A LIVING AND IT IS NOT THAT HARD TO SEND BILLS TO THE CORRECT INSURANCE, ESPECIALLY WHEN I CALL MONTHLY. I HAVE FILED A GRIEVANCE WITH ***** FOR THEIR VIOLATION OF THEIR CONTRACT WITH THEM. I DO NOT WANT TO BE BILLED WHEN THEY DO NOT GET THE BILLS TO THE INSURANCE COMPANY IN A TIMELY MANNER. THEIR CUSTOMER SERVICE AND BILLED DEPARTMENT IS INCOMPETENT. THIS IS IN REGARDS TO BILL #XXXXXX IN THE NAME OF MY HUSBAND *** ******* FOR OUR DAUGHTER ******* ****. DENTAL WORKS GREAT LAKES DENTAL PO BOX XXXXX INDEPENDENCE OHIO XXXXX XXX-XXX-XXXX

Desired Settlement
I WANT THEM TO CORRECTLY BILL MY BILL # XXXXXX TO ***** INSURANCE #WXXXXXXXXX ,AND I DO NOT WANT TO BE RESPONSIBLE TO BILLS THAT DENY FOR TIMELY BECAUSE THEY DO NOT KNOW WHO TO BILL APPROPRIATELY

We have written off the balance of $521.00 as a courtesy for the fillings. Patient was not informed of the upcharge for white versus silver fillings.
