              Ref ACCOUNT# *****: My insurance is ********** ** *******, I called to make a appointment and informed them that I had ********** insurance and asked if they accepted and they said yes. I also checked the ********** website for the provider to insure that he accepted ********** Indiana and it shows they do. When I went in for my appointment I was asked to fill out some paperwork and for my ID and Insurance card, at NO TIME did the staff say Mr. **** we don't accept ********** Indiana we only accept ********** Kentucky. Like I'm supposed to know they only take one version of it. Supposedly they just recently stopped taking ********** Indiana because they said they had a hard time getting them to pay?! If there was a Issue I should have been advised right then I was seated in the waiting area for a good 30 minutes before I was called back for my appointment so there was plenty of time for them to advise me that they no longer took my insurance before preforming the procedure.
