            On Monday, September 20th, I had a scheduled appointment with ******* to have blood taken to test my blood type, as well as for the COVID Antibodies test. I selected ******* for my lab work because they are "in-network" with my insurance company. When I arrived at *******, the representative collected my blood, but stated that I would need to order the test through "requestatest.com". She also stated that I would be reimbursed through my insurance company. I contacted my insurance provider and they gave me the following list of information needed in order for them to reimburse the $45 that I paid: * Diagnosis code * Description of services performed or a billing code * Date of service * Federal Tax Identification Number (from treating doctor or facility) * Billing and servicing address * Receipt showing payment I emailed "*********************" on 9/22/21 requesting this information so that I could be reimbursed. They responded on 9/22/21 and stated that they cannot provide the info

            Chauntae ******* with customer service is awesome as well as her colleague who I didn’t get her name. I had a test scheduled with LabCorp and the employees were totally clueless. First, they assumed I was getting a urine drug screen and told me I had the wrong order number, this is when I spoke with the first young lady who cleared the matter up. She was knowledgeable, patient and sweet. I sure wish I knew her name. Then, instead of LabCorp collecting a blood sample they insisted on the test being a urine. Later, I spoke with Chauntae who set me up to get retested, free of charge. Shes so sweet and even followed up with me a few days later. This company has A-1 customer service which makes patients want to return for services! I sure will! Request a Test give those young ladies a raise, they deserve it! And you should be able to pull up the first young ladies name who helped me in your system, she deserves this praise attached to her name, thanks!
