Complaint: In December of 2012, Lazarus Financial posted a collection account for $1434. This is not a valid debt and no money is owed to either Wheatland Dental or to Lazarus Financial. This action has been electronically disputed with all three credit reporting agencies and proof provided to the collection agency, Lazarus Financial. Unfortunately, Lazarus Financial denies the dispute, confirms back to reporting agencies that the debt is valid and will not remove it until Wheatland Dental says it isn't valid. Summary of Issue: Wheatland Dental provided dental service for ****** ***** under the United Concordia insurance agreement. The insurance company, United Concordia, paid the contracted rate in full for the services provided. The letter from the insurance carrier, United Concordia, states that Wheatland dental was paid for the services provided by the contracted amount of $1530. I have from my insurance an Explanation of Benefits that each of the three procedures was to have written off $390 totaling $1170. With the extra collection charges added on by Lazarus Financial $313.66 this totals the amount of the collection action of $1434 (minus a $50 deductible applied). This account should have been closed in December of 2008 as paid in full. * I have provided Documents of Proof attached that the collection amount is invalid: o Explanation of Benefits from Sept XX XXXX - note remark Q1030 below stating they have agreed not to bill for the difference. o Statement of account showing no action since 2008 on the account until turned over to Lazarus Financial in late 2012. o I have a fax sent to me on June 13th, 2013 copying me on the May 3 letter to Dr. Zmick from the insurance company stating her contracted obligations and confirming that the debt is invalid. Summary of Action YTD: This issue has been contested together with the insurance company for the last five months. Proof was provided to the collection agency that the amount delinquent should be written off as a contracted provider with United Concordia. United Concordia has sent letters and at Wheatland's request, provided the proof of contract that Dr. ***** was a provider in 2008. Personnel turnover in Dr. ******** billing office compounds the problem and Dr ***** refuses to write off the contracted amount. The latest tactic is that they are stating the contract United Concordia provided doesn't look like Dr ******** signature. Request: We are formally requesting all three credit bureaus remove this action as an invalid collection. Currently United Concordia's Provider Relations department is trying to persuade Dr. ***** to write this off as they should have done 5 years ago. This substantial collection amount is negatively impacting **** *****'s credit rating. We feel that we have reached the limits of what can done before taking costly and unnecessary legal action.

Desired Settlement: Complete and total removal of all collection activity from my credit record, a letter of resolution of the error and for Wheatland Dental to write off the overbill amount above the amount insurance already paid including the collection fees added on.
