Programs

              

Since 1985 Care Alliance has served the uninsured, the homeless, the chronically ill, and the disabled through the provision of primary health care. In 2000 we expanded to serve residents of public housing and in 2002 dental services were added. In 2006 a new program was added to provider HIV counseling and testing within the County Jail. With three clinic locations and outreach to seven homeless shelters or treatment centers, in 2010 we served 9,500 people in over 34,000 medical encounters. 88% of our patients are uninsured and most live well below the federal poverty line. Two-thirds of our patients are male and three quarters of our patients are African-American. Most of our patients have co-morbidities including hypertension and diabetes. We provide chronic care management for this hard to reach population. Care Alliance is the primary provider of healthcare to individuals and families who are homeless, low-income, and uninsured.
