Programs

              

The Cincinnati/Hamilton County Continuum of Care for the Homeless (CoC) works with agencies and programs serving homeless persons to facilitate program design and support efforts to end homelessness and to secure and administer appropriate funds to be used to end homelessness in the city of Cincinnati and throughout Hamilton county. Agencies involved in the Continuum of Care Program include area outreach programs, emergency shelters, transitional housing programs, permanent supportive housing programs, and specialized service-only programs. The Homeless Management Information System (HMIS) Program collects data on homeless persons to support planning and increase access to systems of care. The Central Access Point Program includes a telephone service which serves as a central intake point for several local shelters. The Housing Prevention and Rapid Re-Housing Program (HPRP) is a short-term program (up to 3 months) designed to assist individuals and families most at risk for homelessness. This program involves financial assistance for expenses, such as rent and utility bills. For an individual to qualify, he or she must have income or income potential, income less than 50% the area median income, no other financial resources for stable housing, no stays in a homeless shelter during past 2 years, willingness to work with a case manager for up to 3 months, and an ability to remain stably housed following assistance.
