Programs

A Little Bit of Heaven, located in the Roseland Community on the far Southside, provides Emergency Overnight Shelter and supportive services to adult single homeless men in the Roseland community and the Chicagoland area. The targeted population includes Veterans, Ex-Offenders, Sex Offenders, Mental Health, Alcohol/Substance Abuse, the Disabled, those with HIV/AIDS and the LGBTQ family. A Little Bit of Heaven is very vital in the Roseland community due to the lack of social service providers and housing opportunities. With the express purpose of providing services under the Plan 2.0, the proposed project serves as a channel for clients to move from homelessness to 5 beds, 50 designated for the Homeless Single Male population. A Little Bit of Heaven has served the Chicagoland area, in the Roseland community, for 14 years. Our program has served 316 unduplicated clients for 2013 alone. From the time of entry to departure, clients will receive individual assessments,(ISP) service plans, and goals to serve as a guide for their rapid re-housing. Via assertive case management, clients will receive client-centered services as well as connections to community-based programs that will help in the successful transition to permanent housing and self-sufficiency. Our program proposes to serve an estimated 400 unduplicated clients throughout the year From the time of entry to departure clients will receive individual assessments, service plans, and goals to serve as a guide for their rapid re-housing. Through the use of assertive case management, clients will receive client-centered services as well as a connection to community-based programs that will help in the successful retention of Interim, and or permanent housing and assist in the achievement of self-sufficiency. A Little Bit of Heaven acknowledges case management as the key to the movement of clients from homelessness to permanent housing. All clients upon intake will be triaged and assessed for all immediate assets and risks. Once risks are identified, an Individual Service Plan (ISP) will be created to address each risk and provide client-centered steps needed to achieve the identified goal. Referrals are then made to an appropriate provider for services needed to accomplish each goal. Our facility with much experience operating as an Emergency Response Center in the past, the agency is very familiar with clients of special needs. As an Overnight Shelter Program, our facility will admit clients with various special needs. We have informal linkages with the local public Mental Health Facility. Assessments will also be completed for Income entitlements and referrals will also be made to; Employment Services, Public Aid, Social Security Administration, Veteran Affairs, Illinois Department of Human Services, etc when appropriate. All clients are linked with community-based case management services to increase the probability of housing retention up to 18 months after initial placement.
