Complaint: I purchased a tanning and laser hair removal package from Forever Young For $1000. I think it was back in 2011 . I don't know the exact dates .Because I have switched banks since and cant find the transaction. After purchasing the package. I have used some of the tanning and 2 treatments of laser hair removal. I would generously estimate I have used about $200 worth of the total package they had sold me. In july of 2013 Forever Young shut their doors and disappeared leaving no way to contact them. Because of this I have not been able to get a refund for the products I purchased.

Desired Settlement: I just want justice for me and everyone else who is going through this same problem. I would say I am owed $800 but getting anything back would be better than nothing.

Complaint: I have notified Foevere Young over the last 2 years about a refund for a service I cancelled the next day after purchased. I have spoken to their "corporate" office over 5 times about this refund and sent 2 certified letters. I was told to wait 30 days to see if show up on my credit card but it never came. I have called them over 50 times in 2 yrs.

Desired Settlement: I want a full refund as promised of $750 dollars. This is insane that I have had to track them down and plead for my money back when I never recieved treatment and cancelled in less than 48 hrs of purchase.

Complaint: My services where prepaid in full for tanning, spray tans, laser hair removal and facials. The company closed down without refunding me the money I paid for services that where not completed. Multiple times I had tried to make appointments for treatments. I would go to my scheduled appointment and be told I did not have an appointment. When I had and they could not tell me why it was missing. Other times they would call me and cancel saying they needed to reschedule because the technician was out sick. I was never able to keep the scheduled maintenance of the hair removal that when I purchased was told after 6 treatments I would be hairless. When I would be able to get in and not be jerked around sometimes they would have to book out the next session over 2 half months making the treatment I just received pointless because it was past the time frame for next treatment. I have a lot of tans, spray tans, microdermabrasion and compete leg, underarms, and brazilin laser hair removal that I paid for and have not been able to use or finish treatment since they closed down with out notice.

They started numerous contracts for services and consumers (I) have no way to obtain the services I have paid in full for. I have emailed the company asking for a refund and of course have received no response from them. I have copies of original papers signed in there business that state how much I was asked to pay and exactly what services would be included. Complaint: I bought a laser hair removal package from Forever young for $1,600. The package included 6 treatments. I received the one initial treatment but was unable to receive the subsequent treatments because the Prineville branch had closed. I've tried contacting Forever Young by phone and email but all numbers have been disconnected and no one has replied to my emails. In addition to not receiving the service I paid for, there have also been reports of illegitimate business practices made by Forever Young. According to KATU news, Forever Young's owners ******** and *** **** asked and paid employees to write false reviews in attempts to lure people to believe that they were a legitimate business and cared for their customers. It has also been reported that their hair removal treatment is not permanent as stated on their websites. Forever Young was a scam, and that is why they were forced to close their doors. What Forever Young has done to me and it other customers is illegal. They have stolen our money. Complaint: I spent $500 in 2011 for a package of microdermabrasion treatments and never completed all of my treatments. Whenever I would call for an appointment they always said they were booked out for 1-2 months and was never able to get in for treatments. I bought 10 treatments and only used half of them. Then in April/May 2012 I purchased a tattoo laser removal package for $800 for 8 sessions and never got a single treatment. I would get the same response when I would try to call for appointments they would tell me they were booked and I could never get in for my treatments. I traveled from Eugene to my appointments. I received half of the rx I paid for. In April 2011 I purchased another Laser Skin treatment plan. I paid $266 a month for 6months for a total of $1596. I did not get any of the treatments under this new plan. I was unable to travel to Hillsboro due to chronic illness. I was reassured repeatedly that my treatment plan would be there waiting for me whenever I was able to travel again. In the past 6 months I called several times to set up appointements and was told someone would call me back to set up the dates and time. I did not receive return calls. Today, October 22, 2013 I went online to look up their phone number and found out the were no longer in business. Is there anyway I can get my money back?_____

Complaint: I purchased a $3000 package from Forever Young LLC for laser hair removal. The initial plan was to pay approx. 200 dollars a month until I pay off the amount for my services. Throughout the entire year and a half that I dealt with Forever Young LLC I was mistreated. My package included 9 treatments for FULL BODY laser hair removal, as well as 2 year unlimited guarantee after my treatments. Forever Young LLC failed to schedule, or make an effort to set up appointments for me. I received 2 treatments in total, and they were not full body. One was done on my lower arms and above my lip, the other on my lower legs. Combining the total treatments given to me, I received less then half of what should be one full body treatment, and my package included nine full body treatments.Also the technician that attempted to do my lower legs had to stop treatment because she went over my tattoo and burned/blistered my skin. I was extremely hurt and upset. After this incident, I tried countless times to schedule any appointment so that I could receive my treatments. Every time I called, I was given an excuse as to why I couldn't come in. Either their machines were down, they were out of oil, they double booked, triple booked, they're remodeling, I even went into a few of their locations numerous times- in hopes that I would be able to schedule any appointment, but they always denied me. I called the corporate office and spoke with multiple managers and explained my situation. They advised me to use a different location. I explained to them that I had tried multiple locations, and that I'm frustrated with their services,and that I wish to get a full refund since I had not received what I was promised and it had been over a year. I was told that I needed to speak with the general manager. I called her (*****) after a few months of patiently waiting to get ANY treatment I called the corporate office again, and explained to them that I have yet to receive any treatments and that I would like a refund. Forever Young LLC continued to charge my accounts and in total took approximately $3100 from me. I was/am extremely frustrated and felt taken advantage of. Not only did I not receive treatments, I was given attitude, I was lied to, burned,harassed, and Forever Young LLC took my money. I continued to call and email the corporate office and I was advised that I would be getting a call soon to discuss how they can refund my money. Finally, I got through to a manager in April of 2013, and she informed me that she was unable to pull up and discuss my account with me at that time, and asked if I would call her the following Monday so that she has a chance to get my file together and start the process of my refund. That following Monday I called her (*****) back and left her a voicemail. I continued calling her everyday for over 2 weeks leaving voicemails everytime. She refused to answer my calls and actually rejected quite a few of them. Then all of a sudden, their business shuts down and was left helpless and taken advantage of. I have printed all of the statements which include all of the charges Forever Young LLC withdrew from my account. I am a very dissatisfied customer, and I am extremely upset that this has happened to me. Complaint: After purchasing a package originally from the Tansbourne location, valued at around 5,000, I transferred to the Beaverton location based off of SEVERAL cancelled appointments (some of them were my fault too) therefore resulting in the reversal of my services. I have been doing laser hair removal on and off for about 2 to 3 years and now they have closed, and a refund for my packages WAS promised. I was doing laser hair removal as well as body treatments and my partner, ***** ********** originally purchased the packages (same address) and because the services didn't work on him, he gave the remaining services to me such as more hair removal and wrinkle/elasticity treatments. He transferred them because he contacted corporate and they said all contracts have to be canceled within 3 DAYS. You could not possibly (at least my hair type) see the results in 3 days, seeing as how you have to wait for the hair to start coming back in and some with fall out, little by little. It took about 3 to 4 treatments until I saw results, which were about 4-6 weeks apart. Much more than 3 days. When I brought up the fact about how I was treated with my appointments being canceled, I was offered a "free" service. Not only were they usually double booked, but my time was taken WAY down, to about 20 mins for my whole face and arms, when I would request an hour and these requests went nowhere. Now, since they have closed and my tanning package MAY have been transferred to Tan Republic, they promised a refund and quite frankly, this is just as bad as identity theft. There are several news articles that have cropped up about their questionable business practices, and I personally have been on the receiving end. I paid $2,000 over a period of 11 months starting on March 19th 2012 with a $300 initial payment and then a monthly recurring payment of $170 for 10 months for a laser hair removal package that included another 2 years of services that I have not nor will ever receive. I have seen absolutely no results despite having had an amount of treatments that was guaranteed by employees to show definitive results. Complaint: I purchased two contracts with Forever Young, one contract for the amount of $3750.00 on April 11, 2011 which I paid in full. As of today, Forever Young has only honored 1/3 of my services. Everytime I called to make an appointment I was put on hold for 5-10 minutes, sometimes I never got my phone call answered. Making appointments was extremely difficult because they had very few time slots available that were always at least 3-6 weeks out. There were many times when Forever Young cancelled my appointment due to equipment mal-functions including a $1,000.00 Pixel appointment I had scheduled on Feb. 19, 2013 at 11:30. Forever Young cancelled this appointment the day before and I was never able to reschedule this appointment due to the equipment not working or lack of appointments available. Of the remainder of my contract I received 1(one) Chemical Peel of 12 that I purchased, 5(five) Accent treatments out of twelve that I purchased, and 5 (five) AFT treatments of 12 that I purchased. These services cost me a total of $1,750.00. In addition, when I did arrive for an appointment I had to wait for my appointment an additional 30 - 60 minutes over my appintment time. Also, on November 16, 2011 I purchased an additional contract in the amount of $2,000.00. At the time I was told the employees were having a contest to win a trip and that if I purchased this contract it could help them win. I declined to buy this contract but after ten minutes of the staff begging me to buy the contract I reluctantly gave in. I was told that the services I purchased never expired so it was a great deal. In addition to me buying this contract I was given 6(six) free SR Face treatments that I could give to a friend which I did and these services were all performed by Forever Young. Of the 12(twelve) Trinity Face/Neck/Chest services I purchased, none of these services were performed due to lack of appointment availability, and of the 12(twelve) Accent services I purchased, none of these services were performed due to lack of appointment availablity. So overall, none of the services that I bought on this second $2,000.00 contract were honored. I am a part-time nursing student, I work full-time as an LPN and I do not have a lot of extra disposable income. I bought these contracts soon after my husband passed away with Life Insurance money to pick myself out of a depression by trying to make myself feel good. Of the services I did receive, I was happy with the results of the AFTs, and one Pixel, but never saw results from th Accent treatments.

Desired Settlement: I am seeking a "1,900.00 refund for services not performed/honored on my first contract and a $2,000.00 refund for services not performed/honored on my second contract. This comes to a total of $3,900.00. Thank you.

The first package costed $2000, the second costed $500, and the third costed $1200. I had 1 payment of $192.50 remaining for the first two packages so I paid a total of $2,307.50 however, I only received a quarter or 25% of the treatments puchased under the two packages. On July 23, 2013, I decided to purchase a package for my daughter and put a $200 payment as a down payment and a week later, they closed the Spa and went out of business. My daughter did not receive any treatments so I lost the entire $200 that I put down for her treatments. I don't understand why they sold me a third package a week before they were closing down their business. I did not receive any notification from them or anything. They cancelled my last appointment without telling me a reason for cancelling. As per Oregon Law, I would like to be refunded my money. I have copies of all three packages and all my bank statements showing the payments I have made with the exception of 2 cash payments that I made which I am still trying to locate the printed receipts that I received from them. She told me she would speak to ***** who handles refunds at their corporate office and I should receive my refund in 3 days. I called ***** myself and she said that she did receive my letter to cancel and that it would take 14 days, not 3. I called ***** everyday to find out the statis of my refund and she would tell me that she would speak to the owners herself about getting my refund, as she was in the Beaverton location that day when I purchased my package and she remembers me specifically because I had paid part of my package(800.00) in one dollar bills. I continued to call on a daily basis, with my no return calls back, numbers were disconnected, and simply no response at all to my refund. I went back to the Beaverton location to speak to **** and was told that she had moved to a different location. On July 29th 2013 I made a call and spoke to ***** (she told me that she was the store manager) and was given a great quote for laser few areas of my body and was told to pay same day as she won't be working tomorrow and she is the only one can authorize the transaction of $600 and I was also told that if I pay cash she will knock down another $100. But unfortunately I was not able to go the same day but I did went next day ( July 30) with $500 cash. That day a girl who helped me was crying a lot when asked she told me that it was her third day at work and is experiencing lot of problems as most of the tanning beds are not working and she had to answer the customers and was getting yelled at. She was not able to give me receipt as she did not know where receipts were kept and I was told that she will ask ***** to make one and she will call me. A week into it I never got call from ***** or any other person working there. When I called asking about my receipt someone told me that receipt will be kept at the front desk and they will keep it there and suggested me to pick it up when I come for my Ist session which was on Aug 14 at 10pm. Since, it will be first session with them I called them on Aug 13th but no one picked up and there so I called again and again no answer phone kept ringing. Complaint: I have been a client for over 18 year with first **** *** and then Forever Young. My husband ***** ********** and I both have tans and Laser hair removal contracts with Forever Young. I have the q-switch and pixel services in addition. I have paid in full for all of these services, only to show up to the site without notice and find that they have shut their doors and refer the clients to the ***** *************** location. I went there to find that they had also closed their doors. I don't know about you, but that is a definite breach of contract for services. We spent quite a bit of money for these services and had been very dedicated clients/customers. I guess we expected the same back from this business. No notice or anything.

Desired Settlement: I would expect that they would provide a refund of money for the agreed services. I paid in full for all of my services in which the contract of services was broken. Isn't that what they would expect from me as a customer. If I signed a contract with them, whether I like it or not, I would be responsible for agreed monies for agreed services.... Several excuses were given like machines would break down, lack of availability of appointments, technicians not showing up, always running late and canceling appointments after waiting for up to a hour, false information and SEVERAL times they would stop in the middle of treatment because they were running late to add you to a waiting list that never existed. They would of course take your signature down to show you went in for treatment that day although it would be incomplete or not even started.

Desired Settlement: I am asking for a FULL refund of all the money I paid and would like to request they pay me for my time, gas/mileage, frustration for the numerous times we showed up just to be sent back with no treatment.. we did not get any results and not ONE of the packages were ever completed. Complaint: To Whom It May Concern: I am sending this correspondence as a means to file a formal complaint against the Hillsboro, OR based business, Forever Young. I read a story, dated July 31, 2013, posted on *** *********** **** website indicating that Forever Young closed its doors after an investigation by the State of Oregon. Their closure is the nature of my complaint. I have been a customer of Forever Young for over 7 years, the last three of which, I conducted most of my business with them at their location in Tanasbourne. Over the course of that time doing business with Forever Young at Tanasbourne, since 2011, I have spent $5,139.61 on services from Forever Young. Most of these purchased services have not been used and, now with Forever Young's closure, are not redeemable. I made an attempt to confirm the services remaining on my account at Forever Young on several occasions and was consistently rebuffed by their excuse that "their systems were down", or "they needed more time to research." Finally, I was able to get an employee, *****, presumably the Manager of the location, to complete a list by hand, as she explained that they were "currently unable to generate a printout but that she would copy the available open services from the system onto paper for me." The listing that ***** compiled that day is attached. I have read that tanning services that were available at Forever Young will now be available at *** ********* However, I have received no such formal notice from either Forever Young or *** ******** as yet. Given the closure of Forever Young and my pending, purchase, and now unredeemable open services, and no indication from said business as to what steps they will take to make me whole on the out- of-pocket expenses associated with the now unavailable services, I would like to lodge a Formal complaint to make my case known and to find out what steps may be available to me to pursue remedies for damages associated from this businesses abrupt closure.

Complaint: I am writing a formal complaint against Forever Young because it seems they have closed up shop without informing their clients who have pre-paid spa packages that they are going out of business. They have no contact information and it seems no intention of fulfilling their obligations to those of us who have paid for services we have not yet received. I have paid for all my full body laser hair removal treatments in full. Forever Young had horrible service and the treatments were not very effective, but the complaint I am filing is because the business is closed and no compensation has been made for the time and money I spent on services that were not even close to completion.

Desired Settlement: I had laser hair removal for face, neck, underarm, belly, bikini, legs, buttocks, and arms, nearly a full body package. I had a greatly "discounted" package and spent $1,442. Unfortunately, with the nature of hair removal, it takes diligence and timing to catch hair in the correct cycles. Forever Young was always "losing" my appointments, something wrong with the machines, and then couldn't reschedule me for at least a month on end, so the results are pitifully lacking. I have wasted almost a year and money I didn't have on something that didn't work. I expect to also be compensated for my time so I can afford to go someplace that will give me the results I deserved in the first place.

I have been paid in full for almost 1 year now. On my first treatment they burned my upper lip. I requested a refund but instead they added at least 50 other treatments to my package. This establishment has terrible customer service.Not only do the machines breakdown and you are not called in advance but they often run out of gloves. I would also say that they run late about 90% of the time. They are consistenly overbooked. More so, it seems that they have lost my chart on more than one occasion.

They said that this information would be posted on their website. However, by Thursday 8/1 the website was deleted and all the phone numbers cancelled. I have just lost any way to contact the company, and I've lost all the tans and laser treatments that they confirmed the day before. I was unable to use the services due to their schedule and then they even got rid of some of the equipment. In May of this year I asked a manager to speak to **** or someone at Corporate so I can get tan credit for my thousands of dollars in spa services. I was told that wasn't allowed and they couldn't do anything for me. I emailed someone from the main web page as the manager REFUSED to give me corporate contact information. A few days later on May 24th I received a phone call from ***** ******* She stated she was a corporate officer and wanted to help me. I explained what was going on. Instead of a refund she transferred my spa services to tans. I had used a couple through the summer, but in the last 2 weeks I tried to use my spray tans with no success. First they told me they were out of solution. Then another time they told me a part was missing. On Monday, July 29th, I had had it with them. I emailed again through the website and also to **** requesting my monies returned to me. I asked for the full retail value of my remaining services. I even attempted to contact corporate again with the phone number that ***** had given me. Her message stated that was no longer a line for Forever Young and gave another number to call. Upon calling that number I learned the voicemail box was full and wouldn't except new messages. As this point I have not been contacted by Forever Young about my refund.

Complaint: Forever Young has decided to close their doors. Yesterday afternoon they charged my card for a monthly fee knowing that they will not be honoring my unused package. Complaint: I dont even know where to begin there were so many it started with ***** the sales person he used tactics / high pressure to sale a package he stated he didnt have nay sales all day so he would sell me a pkg for 1000 for 24 sessions and he liked me so much he would allow me the friends and family discoun - I called the "corporate manager" ***** to discuss my concerns and requesting a refund she would not refund me after telling her my expiernce I referred to it as diress as that is how I felt. I also spoke to a ***** at the Beaverton location as I expressed my concers she informed me I would not be welcome in the spas for voicing my concerns, I made SEVERAL calls to ***** with no resolution-also getting poor service, on hokd, didnt return calls I expressed that I wanted a refund as I was so un comfortable witht his exoerience - she refused and made 5 appts for me each and every one I recived a call at 8 PM the evening before my 8 AM appt. to say all the equipment is broke or we over booked I was able to use 3 of my sessions as I felt I had no other choice - the last time the tech burned my skin I continued to be very un comfortable and stating I cant handle this heat she said she was new and was tought this way, this was the worst expierence I have EVER had - and I would like a full refund! as thier tactics were shady, scetch, unprofessional and thieft!

Complaint: My husband **** and I signed up for laser hair removal back in 2010 and then again in 2012 for additional areas. When I first signed up in 2010 the areas that I purchased where upper lip, under arm, and full legs. This cost was $999. Then later in 2010 I purchased the Brazilian treatment and **** purchased the full back treatment, again adding additional cost. Then in 2012 **** signed up for his head, face, and neck, and I was offered a complimentary area since we had paid for so many locations. I opted to have my full arms done. The cost for the face, neck and head was about $750 if not more. We have spent close to $2,000 on laser hair treatment and they continue to cancel our appointments, the company closed the Hillsboro, Oregon location down without even giving us notice, we switched to the Scholls Ferry location in Beaverton and now out of the last 3 or 4 appointments that we have scheduled, they have only gotten us in for one appointment, which was the very first one over 7 months ago. In the last 7 months the company has cancelled, rescheduled our appointments without our knowledge, has cancelled our appointments without telling us, has put us on hold and left us there for over 15 minutes forcing us to call back just to be put on hold and not helped. I have attempted calling their corporate office at (XXX) XXX-XXXX several times. The first person who took my information about the complaint said she would get back to me in 2-3 days, when she did not call back, I called back in looking for her and her update, and was told that she was fired and that I was speaking to her replacement. The new employee took my information again, said that she would have a manager get back with me, and that was over 2 weeks ago. I have tried calling back, but no one answers the phone and when it goes to the voicemail it says that it is full and I can't leave a message. I have left several messages before the voicemail box was full and no one returned those calls either. I was told when I purchased the laser hair that I was the perfect candidate for removal and so was my husband since we both have brown/black hair, it is thick and course, etc. I was also told that the treatments where guaranteed for 2 years after my treatment was completed and I have been attempting to have the hair removed for over 3 years now and none of the areas that my husband and I paid for are finished. I was then also told by the first person who helped me several weeks ago that I was running out of sessions. When we purchased these packages they were not based on a session amount, they were based on the hair being removed completely regardless of how long it took. Now I am being told something completely different and it is in breach of my contracts that we have signed. The company could not even locate my information when I called their corporate number for help. They were able to locate my husband **** ********, but they could not locate me under my married name ******* ******** or my maiden name ******* ******* We have given them our updated information each time we have moved and when we got married and it seems like they are not keeping their records updated either. When we would visit the Hillsboro location that is now closed, the machine would always be down or broken. Our technician *****, ******* or whoever was working that day would always tell us that is was overheated or that they were being sent a new one and we would have to reschedule. In over 3 years we have not completed anything that we paid for due to their scheduling issues, machine malfunctions, and lack of customer service.

Complaint: I was promised certain treatments would actually "fix" different cosmetic issues by the salespeople. On more than one occasion once I got in the room with the technician and they told me the results I'd get were probably not what I'd been promised. One of the packages I purchased -- a pixel package -- the tech told me would damage my skin since I tend to hyper pigment. The people who talked me into the package (***** and ****** were singing the praises of pixel and how great it would be!!! Of course they weren't the ones treating me and my laser tech said, "Oh, I wish you'd asked me! I would NEVER recommend this for your sensitive skin!" This was *** ** She told me that night was her last night with the company. I was very sad to see her go. I was told I could not get a refund, but could trade it for other services. I wish I had fought for a refund, but then went ahead and traded treatments. They were things I never really wanted in the first place. I tried to get out of a package I purchased last summer (June 2012). I followed the procedure outlined on the contract by sending a registered letter within the three days. I got ahold of one person, who said that she didn't know where my letter was, but that the office had moved and it was probably in a stack. She assured me I'd be contacted once it was found. I then asked at the Beaverton location and ***** gave me a false fax number when I explained what I was doing. I told him I was faxing the proof over to the administration office to show I'd followed the required steps. When I still didn't hear, I asked again at my next appointment and was given yet again a different fax number. I understand they've closed their doors, but I still have unused things that I have already paid for.

Desired Settlement: I would like to received some of my money back. Laser hair removal did not work -- still growing hair in areas treated. I also have unused treatments left in my package that I have paid for. I also felt it was important to add my name to the list of customers who feel they were deceived by salespeople trying to earn trips to Hawaii, and promising things that they knew never would be fulfilled. Complaint: I began going to Forever Young spa 10/5/2012 (October 5th, 2012) for cellulite reduction treatments. I had a friend who worked there, her name is **** ******* She said she could give me a friends and family discount and that my payments would be around $60 a month. I asked her what the package included and all the policies they had and she said she would email me all the information along with one I had to reply to in order to accept the contract. I got an email but it did not have any information about my package, payments, policies, etc. **** told me the initial payment I would have to make is $200, which I asked to split up in the 1st month. But they charged my card on 10/4/12 for $150, then on 10/16/12 for $170 ($20 more than what was supposed to be charged). I brought this up to **** and she said she would fix it, but I was never refunded. On 11/15/12 I was charged $170 when I was told my monthly payment would be $60. I complained again to **** asking her what was going on. She said she would talk to her manager and fix it. Again no one contacted me to let me know what was going on. On 12/3/12 I was charged 3 times as follows: $170, $170.01, $60. I contacted the corporate office immediately and asked to be refunded. But then on 12/15/12 I was charged another $170. This was infuriating because I was almost to my credit limit and days before Christmas. I couldn't do any Christmas present shopping until they finally refunded me for the December charges on 12/20/12. I asked **** to cancel my package and remove my credit card number from their files. She told me she sent a detailed email to the owners via text message and was waiting back to hear about what they are going to do to fix this. I never heard the response from the owners, but **** said they would pull the rest of my payments and my package would be free. To me this meant they wouldn't be charging my card anymore. I didn't even come in for anymore treatments. All I wanted to do was to pay for the treatments I received and cancel my package. On 01/30/13 I was charged on the same card for $63.75. I was given the corporate office number so I called and asked them to look over my account and all the notes and please take my credit card number off file. They said they would review it and call me back within 24 hours. They failed to do so. I called back again and they gave me the owner's cell phone number ***** and I called and left her a detailed message about my account and situation. She called back and said she would review it and call me back within 24 hours. She never called back. I have called every week since then and have been told the same thing every time that I would be called back within 24 hours. Not once have I received a call back. And every month this year I have still been charged $63.75 every month this year. I have every phone call and text message documented, and every **** credit card statement with all details.

Complaint: On February 4th, 2013 I bought a $1,200 Laser hair removal package. I ended up paying a $200 down payment on my debt card as my first payment, and the remaining $1,000 were to be paid as monthly payments of $100 that would be automatically deducted from my checking account on the 15th of every month until December 15th 2013(which I have emails and bank records). My package included laser hair removal from my hip all the way to my face(half upper body). I was guaranteed by the manager ***** ***** I would see instant results, and it was a painless treatment & that each session had to be 6-8 weeks apart. My first session they took me into a room were I had to unclothe myself and after I was done and before I had walked out the room, I noticed the doors had a bit less than 1 inch between them. Which I began to feel uncomfortable not knowing if employees and customers could see me half naked because the doors didn't create any privacy. After every short treatment I had, their politeness started going down the drain. Continuously I kept getting booked not enough time to even get barely any treatment done. A few times they canceled my appointment upon arrival because I was 5 min late. On my last appointment at the end of June 2013 the technician had increased the power of the laser, which then started hurting my skin I left with watery eyes and customers starring at me because I believe they heard me moaning and yelling about the pain I was in. I had shown my bf and my skin had broken out in red bumps and it was tender to the touch. Weeks later I noticed my Hair Line was all crooked. I had missing hair on my forehead from my actual scalp. Then eventually I noticed my back neck hairline was all crooked. Now I am left with extremely bad results, and I have pictures & myself as evident proof. My body has been left with more than emotional scarring, but now I have random patches of hair on my body, and missing hair from my scalp and a crooked hairline. Before all of my mistreatment, on May 11th I had also purchased 10-Q switch full face and 10-Micro-dermabrasions. The treatment was suppose to help lighten my skin tone and help remove dark brown & red spots on my face. I had talked to the manager from the Hillsboro Forever Young location who was at the time covering at Beaverton, her name was ****, who also said I would notice immediate results after my first treatment because she too had the same services done on her. I bought my package for $500 on my credit card because I was convinced that they could help me because I thought they were going to be professional about their business. I then scheduled an appointment, which was suppose to be on the first week of June(a whole month after purchasing my package). Come to find out I get a phone call that the specific machine that was suppose to do my facials got sent back to the manufacturer because the machine was having problems. They had told me it was going to be a few more weeks until the machine came back and they would book me ASAP when it came back. After calling and checking in, then the receptionist finally mentions that they have another location were I could go to and get my facial treatments done at the Forever Young **** ** ***** *** Hillsboro, OR. 97214. My first treatment since purchasing the package on May 10th was June 30th at 6:45 pm which was also my last treatment. I walked out of there seeing no change on my skin tone, no immediate result, not even the slightest change. That day I started noticing how they kept pushing their sales on me and how they would offer me free trials of other laser treatments before, during and after my appointment- all in one day. I also have voice mails I got on 6/29/13 from ***** and another on 7/12/13 ******** both from the Corporate Office wanting to talk to me. After I had returned their call around 07/20/13 I spoke to a women who said they were having this special were if I paid the full amount of $500 I still owed from my hair laser removal treatment I would get $100 off. Luckily I refused and said no.

Desired Settlement: I want justice to be served and a full refund from a company who clearly did not abide by their own contract and abused their written promises. Now I am left with paying the price of a huge problem that I went out to seek help for and was mislead, and I am left feeling worst about myself then before. They need to fix the wrong they have done to me and what they robbed me of. I am asking for a full refund of my $1,200 I paid of my hard working money. So far I have paid them for the laser treatment $200 down payment on Feb 4th + $100 each month for 03/15/13+ 04/15/13+ 05/15/13 +06/15/13 +07/15/13 =total of $700 + $500 for the Micro and Q switch facials= $1,200. I went there to feel more confident and to boost my ego, and they ended up doing more damage than anything. Now I have to go to my Bank and freeze my account because I am not sure if they are going to continue stealing my monthly payments. Complaint: We went to the Hillsboro location to gather more information around Lazer Hair removal for my wife. They informed us that this was a prepay option only. We began with a payment structure then received a call offering a small discount if paid in full. We opted for the pay in full option. We have experienced several frustrations with the service provided at the Hillsboro location. On several occassions employees have called to cancel the appointment while my wife was either in route or in the parking lot. There have also been several occassions (last cited 7-1-13) that her appointment was cancelled once she arrived. The reasons for cancellations are usually not enough techs working. It is also difficult to make an appointment by phone as they usually leave you on hold, disconnect or say that the person who makes the appointments is unavailable. We are unhappy with the results as the amount paid was for hair removal and the hair still remains. The only reason she has continued to go is that full payment for services has already been received which is likely why the management does not return calls

Desired Settlement: As the treatment results have not been finished and there hasn't been care from management to respond a partial refund is requested of $600.00 which represents half of the amount paid. Complaint: I paid in full for a facial laser service for a total of $500. This was back on April 16th, 2013. Only a few hours later, I decided to cancel on the contract due to financial issues. I am a graduate student, and realized that I could not afford to spend $500. I called to cancel my contract with Forever Young, and sent them a written cancellation letter sent to their main office via certified mail. The main office received my cancellation letter, and told me that I would see my refund back in my account in a few days. It is now July 1, and I have been calling the corporate office of Forever Young each day. Every time I call, the corporate manager does not pick up, or if she does, tells me to call back in a few hours (which then she does not pick up). I have been calling everyday for the last couple weeks and still have no idea where my refund is. At times, the manager will tell me that she will give me a call back once she figures out what is going on. I have never received one call back from Forever Young. The corporate manager even told me 3 weeks ago that my money would be in my bank in the next 2-3 days. I still have not yet got my refund. I followed all of their cancellation policies/rules, and still am confused as to why I have not yet got a refund. I am beginning to get very frustrated with the service that this company is supplying. I

Complaint: Forever Young has repeatedly dropped my appointments from their schedule and failed to reschedule me in a timely manner -- if at all. Recently I was told that the manager would call, she never did. Then I was told that the person on the phone was unable to work the computer enough to even tell me if my appointment was still standing. Another call resulted in me being told that the person was unauthorized to schedule an appointment. In addition, I have been going for laser treatment on my legs for three years. Initially, I was told that 6 treatments would result in complete hair removal. Though most of the hair is finally gone, it is by no means complete. I wonder if the laser technicians were really "zapping" the hair at full power or if I was just being kept primed for more sales opportunities for them. Every single visit required that I listen again to some hard sell on a service.

Complaint: I went in on Friday, May 10th to Forever Young to purchase a spray tanning (Versa Spa) package. They sold me 75 spray tans for $150. I paid in full there and was told I would be able to use my tans whenever I liked. I have spent the lasts 3 weeks calling the corporate number (which is a mobile phone) and not only does no one ever answer, but I am not even able to leave a voicemail because the voicemail box is full. One week ago, I spoke to one of the managers at the salon and explained the situation and asked for a refund and she told me that no one ever gets a refund. I explained that I pre-paid for a package that I am not even able to use, and she basically told me "too bad". Yesterday, they closed the Hillsboro salon location closest to my house for unsaid reasons.

Complaint: My complaint is twofold: FIRST: On 3/29/13 I purchased an "upgrade" to my existing services for $500. I was very hesitant about this purchase as Forever Young has a history of selling packages but then not having operable equipment, enough staff on duty, or time slots to schedule and deliver services. At the time of the transaction, the manager ****** assured me that I could get a full refund if I thought it over and decided against it. She indicated that it would be "no problem at all" for me to get a full refund if I sent a request by certified mail within three days. The next day (3/30/13) I called ***** and told her I'd be requesting a refund. She indicated that she'd likely be in a lot of trouble and that her superiors would be calling to ask her why. The letter was received in a timely manner, yet four months later my refund has not been issued. I have talked to multiple staff members at the corporate office who have sent emails to the accountant and the owner, ***. I have asked that the accountant and/or *** return my call. Neither ever has. I have been told by both corporate managers (both of whom are no longer with the company) that my refund was in process and that my money would be refunded in full (NOT less the 15% processing fee). When trying to follow up, I have been hung up on, lied, to, and referred on to people who have never contacted me. To date, I have not received a refund nor have any of my calls ever been returned. SECONDLY: As has been the case with Forever Young for years, I have tried multiple times to schedule appointments to use my services and am always told that they are "booked up for weeks" but that I'm welcome to call every day to see if there are cancellations. In addition, when I have been able to make an appointment they have been cancelled by Forever Young staff 50% of the time due to machines breaking down, staff not showing up, or double booking. Many of the services Forever Young offers are only effective when done on a consistent basis. When appointments are cancelled, the efficacy of the treatments are nill. Forever Young oversells their services and are not able to deliver what their customers pay for. Staff members have put together a calendar of appointments to ensure that services were deliverd. That has worked in small increments of time, but after 2 or 3 weeks, the cancellations would begin again and the whole calendar would get knocked out of sync. I have MULTIPLE services in my account that I have not been able to use due to their issues, yet whenever I'd walk in to their location I'd be descended upon by staff who'd try to hard sell me, alwasy stating that they have a quota to reach or a competition to win (one of Forever Young's sales/production incentives for staff is plastic surgery). I have given up on trying to access my services as they have not had appointments available when I call. Often times nobody even answers the phone. I am now about to move and will not have the opportunity to use my services even if Forever Young were to reorganize and offer better, more consistent service.

Desired Settlement: First and foremost, I would like to have my money refunded for upgrade purchased on 3/29/13 in the full amount of $500. I did exactly what was required to ensure that my money was refunded and have been informed by staff that my refund was received on time and that it was "in process". Forever Young has not even remotely honored their policy and, therefore, I do not think that a processing fee for them to keep is in order. Secondly, I would like to be refunded in the amount of $500 for services that I have not been provided due to the fact that Forever Young is not able to deliver all of the services they sell to all of their clients. They are selling more than their staff, equipment, and space, can deliver. I realize that there is not a firm policy in place for this second request, but out of principal and sheer frustration over the lack of professionality and broken appointments and promises, I feel I need to seek this as well.

A contract would await me at my first appt. I paid the deposit and was told that "financing" would contact me about my withdrawl date for each monthly payment on 5/30. I contacted ****** ***** and ****** (corporate) on 6/3, 6/15, 6/22, 7/9, 7/18, 7/24, 7/26 at XXX-XXX-XXXX and email and left a detailed message/email each time. I never received any calls/emails back. I have records on my phone/email. I went to my scheduled appointment on 7/28 at 5pm only to find a note on the door stating the site was closed and sorry for the inconvenience. I called multiple sites and there was no answer to speak to anyone. I called the XXX-XXX-XXXX number to no avail. I sent an email to ****** ****** account rep at his email as well as through the Contact Us email on their website asking for an immediate response as to why the business was closed, why I did not receive a phone call indicating the business would be closed and to offer to reschedule my appt and to discuss my deposit and billing. I asked for a reponse by 5pm on 7/29. I have not received any response. I called the XXX-XXX-XXXX number at 10:02 am today 7/29 and the voicemail was full for the number. At 5:20pm, I called again and the voicemail had been changed to answer Forever Young Corporate Office, yet the voicemail was full. I called the Tanasbourne location and asked why the business was closed and I was told she was a tech and "didn't know - they were cleaning or something". I have not been responded to by phone, email, or mail. It is clear to me that this is not a reputable business venture and I do not want any association. I want my deposit back in FULL within 14 days. I want a letter indicating that no charges with be drafted from my account and my "account" with Forever Young will be closed.

Complaint: I had trouble with some of the personnel, in that they could not even explain the services they were giving. I could not get them to give me a schedule of my appointments and I asked probably 7 - 10 times. They changed my appointments or did not even do the appointments. The procedures they do is very personal and they kept changing personnel so much I never knew them most of the time. They would tell me one thing about when I could have the procedures and then someone else would tell me that was dangerous. I would go in sometimes and they were standing around and would not get to the customer promptly. I could hear their conversations and it was personal business not business related to the job. I did have some of the procedures and do not expect to have all my money paid back to me which was paid. It totaled $1,595.00 I do expect to be paid back for the balance that was not used. Complaint: I was and technically a customer at forever young spa until they recently closed down. I'm very disappointment because I wasn't able to complete my package deal with them even when it took months for scheduling an appointment with them. I wasn't able to notice any results from those three appointments and now I won't be able to know if what I paid for was what was promised to me when they sold it me. I wasn't there long ago for a upper lip appointment that I scheduled alone from their recommendation that I've only had 4 times not included with the 6 total. Struggling to get appointments with them as they have been canceled multiple times because the tech left or there was no time for me. Especially noticing that my most recent appointment I wasn't given a call the day before, still going in I felt the girls and the tech at the counter were surprised I even came it at the same exact time. Sitting and waiting being the only customer there hearing "not sure if I have time."

Desired Settlement: I would like full refund or at least half of what I wasn't able to get from them because I wasn't pleased with the results and now I won't be since I can't complete the 6 sessions that was suppose to show me the full affect of their laser hair removal.

Complaint: In January I purchased two separate packages from Forever Young. My contract was that I would pay them a set amount for services I would receive throughout the year. My payments were divided up into 12 payments. In Feb, I then purchased another package for completely different services from them in the amount of $1,500. The same day, I changed my mind and called to tell them I wanted a refund. I spoke to ****, who is also the same girl who sold me the package. I called her throughout the day, but was told that she needed to speak to someone in "corporate" to authorize a refund. She never got back to me, and told me she did not have the ability to give me an answer. Funny thing is I have heard her tell other people she is a "manager" and can authorize packages and prices of packages. I read the contract I signed with them on that day (since no one was getting back to me with an answer)which was a friday. I was concerned that if I waited until monday to hear back from them, enough time would have gone by and they would say I no longer qualified for a refund. I followed the procedures on their contract and sent a certified letter stating that I wanted to cancel the contract and I wanted a refund of my $1,500. I know they received it in their office on the following MOnday, because the post office's website stated as much. I never heard back from anyone, and spent the next week calling and calling the Beaverton location. I spoke with ********* and she said she was trying to get someone from corporate to respond but could not. She told me several times that she had talked to the owner ********, but they said they were not going to give me a refund. I sent emails and left phone messages for ******** in the corporate office, but she never responded to me. It wasn't until approximately 6 weeks later when I finally had several phone conversations with someone who identified herself as ***** the corporate manager. She first told me that I did not respond my request for a refund in the specified time frame. When I proved that to be incorrect with copies of my documents, she backed down and said that they couldn't give me a refund because I had continued to use their services (even though I still had 2 previous contracts that I should have been able to use.) I told her that since the dispute, I had not been back into FY for any services. She again changed her story and said I had authorized them to put the $1,500 toward my remaining balance on the first two contracts. I told her that I did not, and wanted the money back. She said no way. I have many issues with them that are small and insignificant compared to the issue regarding money. The other issue regarding money that upsets me is, even knowing they would he closing their doors this week (today is 8/1/13 and they are closed) they took another $235 out of my checking account on 7/29/13. I verified that with my bank. Two times in the past 6 weeks their employees were trying HARD to sell me additional packages. I was told, "it's our slow time of the year, and I can give you a really great deal." Also, ******** from Beaverton called me last week and said if I paid off the remaining balance on my contract, she could give me a discount on the total. I told her no, I don't have the money to do that. She kept asking and didn't want to take no for an answer. Thank God I really did not have anything to give her. They are liars, and thieves. They knew they were closing their doors and would not be able to provide services anymore, and yet they continue to take money out of my checking account. That has to be illegal on some level and certainly immoral on another. I tried several times to get an exact accounting of the services I still had available to me, but they never followed through with that.

Desired Settlement: I'm not sure. I will never see the rest of my treatments. I do not want them to be able to keep taking money out of my account every month. That's just stealing. I feel they have already stolen money from me. I feel completely ripped off. I would like my money back.

Complaint: I had an outstanding balance of $60 and was called. I gave my credit card # over the phone and set 3 payments in place 2 weeks apart from each other. When they ran my debit card they ran it for a full $60 plus an additional $90. I called a couple days later and spole to the same woman who ran my card. She appologized said she would look into it and refund the difference that "someone else must have done" It never happened. I since then called an left 8 messages. No one ever followed up and no money was ever refunded.

Desired Settlement: I hould have all balances due waived since the unauthorized $90 that was charged cause my account to over draw and recieve overdraft fees. I should be credited back the $90 and the $60 as a good act of customer service.

Complaint: In Decemeber of 2012, I signed up for multiple services with Forever Young. Before signing up I advised Forever Young that I am taking Accutane, a very powerful acne drug. Despite being a laser spa who should know that I am not eligible for any treatments due to the Accutane, they disregarded it and signed me up and took my money anyway. After only two treatments since December, I was approached by the same salesperson who sold me my first package, telling me I should quit Accutane for 10 days, and once I did that, I would be eligible for various expensive laser treatments, which would eliminate the need for me to continue with my Accutane, because these laser treatments would eliminate all of my acne and scarring. I chose to sign up for that as well and quit Accutane soon thereafter. About two weeks later, I called to schedule appoinments for my laser treatments on my face. The same salesperson now advised me that I would have to wait at least six months to do those treatments. She then told me she would double check with the doctor and call me back. She did not call me back, I called her back and it was at that time she told me that the doctor said I was unable to do any of the treatments because of the Accutane. I asked for my money back, to no avail, I called the corporate office to get a resoloution, only to have the manager tell me she would begin to process my refund, then a few days later disconnected her phone. I cannot get ahold of anyone. This company knowingly accepted my money and signed me up for treatments I was not eligible for. I am requesting a full refund, due to this fact. There is no reason I should have to pay all of this money for services I cannot use.

Complaint: After just being diagnosed with skin CANCER by a doctor and not really knowing anything about it or how to treat it, I was convinced by the sales team that the treatments at Forever Young would help treat it and solve my problems. I was hesitant and did not purchase the package right away then. I told them that I would need to talk to my doctor and was waiting to see a specialist that takes and good 3 months to see do to the amount of patients they have.. I received approximately 5 phone calls the next few days. Saying that the price would go up if I did not purchase now and that if the doctor did not recommend treatment they would refund my money. After scheduling my first appointment with a nurse she was very concerned with who sold me the package. She said with the list of medications I was on and skin cancer that it wouldn't be a good idea. So after a practice treatment and one other treatment I finally got in and saw my specialist. They told me that I should not get treatments and wrote a letter stating that and then I took it straight into forever young. I wanted to make sure that this got to management and asked to speak with ********. I was told she was not in. The gentleman in front said he could help me so he took my letter and said he would put a copy in my file and call corporate and start my refund. Fast forward 2 years or longer and still no refund. I have tried emails, faxs, phone calls, letters all which where ignored. They have never had a problem in the past with excepting my money for services but once and legit refund was in order I was completely ignored.

Complaint: I purchased 2 packages. 1st was 10 treatments for $600 and the 2nd was 18 treatments for $500. I purchased the package to remove white hair on my face. The first treatment they told me it wouldn't work, but they'd try. The treatment made me sick to my stomach and my heart race. They then said they could remove the spider veins on my face. I only received 8 treatments because they cancelled me 6 times. I have contacted them in person for refund and called the Prineville office, both told me "No way!" My attorney contacted them and they lied (in a letter) and said I had used all my treatments. 3 times they rescheduled me when I had proof of appts, 2 times they called me 5 min before the appt (when I was on my way), that my technician didn't show up, and the last time, the YAG machine had been sent out from all locations, at the same time, to be recalibrated. It would not be back for 2 months. They said they would call me when it came back and never did. By this time I became weary of trying to make this work.

Complaint: Arrived at the establishment for an 8 pm laser appt. I was 3 minutes late & I apologized. I was told it would be a "few" minutes & to have a seat. 30 Minutes later I approached the customer service counter & asked how much longer the wait would be. The young girl sitting behind the counter somewhat snobbishly said I'm sorry....we are running a little behind. That would have be nice to know from the time I had checked in. This is the 3rd time I have waited for appts that were scheduled(Not walk in appts.) I proceeded to sit back down & another customer came in(after driving 50 minutes) for her laser appt; she checked in & was told it would be a few minutes. She heard my conversation with the customer service rep & asked how long I had been waiting. I told her it was now 40 minutes. I was not happy. We proceeded to have a conversation regarding our experiences with this company. The "young girl" from behind the counter approached us & basically told us that she understands we are unhappy but that we can't sit inside & talk badly about the business while there were other customers inside. The only other customers inside were 2 girls & it was quite obvious they were regulars. Both the other unhappy customer & myself interrupted the "young lady" & explained to her that we are the customers & we are unhappy & we have rights as well. She told us we didn't. I have never been treated to poorly from a business. These young ladies that work at these establishments are horrible. They sit around & discuss their social lives while deciding which dance club/party they are going to on the weekends. They have no customer service skills at all. I am a paying customer who for the most part enjoys the "services" alone. I could do without their selfish attitudes. This is suppose to be a business that wants to make you feel good about yourself. While I'm somewhat pleased with my results from the type of service I get; I felt so completely ashamed that my $ pays for that kind of treatment.

I have just finished up a review on google and by the looks of it; I'm not the only one who is unhappy with this company. I would also like a phone call from either a manager or owner with an apology-in person from the young lady I mostly dealt with.

Complaint: This spa asked me to write a check to show my good faith on a package that I was purchasing. I was told by the employees that it would NOT be cashed and I could return and pay in cash. I returned a few days later and gave them money, I was told the check was DESTROYED. After checking my account a few days later I found out they not only cashed it but wrote in the CASH AMOUNT and CHANGED the DATE on the check (FORGERY!!) which caused an over draft and a TON of fees! I have been in touch with corporate ,*****, and they have not been ANY help. They have been giving me the run around and refuse to answer who FORGED my check. I am still dealing with them but basically what I've been told is that it's my fault for leaving the check and they will NOT refund my money. not to mention every time I go in for a treatment it is weird for me I'm getting the cold shoulder and no answers ..***** has told me she was gonna get answer for me. and everyday since the 19th of april ***** tells me to call her tomorrow also she told me ***** had no right to give me such a good package deal for that price. so she was gonna have to take out a few treatments and I was like no way please help me

I would like it fixed since its been a mess for a few weeks and I would like the 2 packages combined and no payment for a couple of months since they already got payment a total of 500.00 dollars was given down. and more payments were coming due i just feel they should be put out since the mess with a check that i was told was not gonna be cashed.I do want this resolved. The original package was cancelled because this was a duplicate package and any money put toward this package has been credited to the other two packages purchased by ****. This was at **** request. I have taken care of this and do not know where else to go with this case due to the fact that this clients issue has been taken care of. When **** bought the package in question, she was offered a promotion we were running that day. **** wanted the promotion price but did not have the down payment. Between herself as well as the employee they decided she would write a check that would not be deposited. She was instructed to bring the cash in the next day or the check would be deposited. **** brought the cash in almost a week later. The check had already be deposited this client rendered services and there was no communication with the corporate office stating the check was not to be deposited. The check was never foraged. We also have a copy of the check, however I am unable to provide a copy on line because this has the clients account information on it. We do not and will not break the law and as a company do not take these false accusations lightly. I will be contacting our attorney because these as well as other false accusations **** has made. When **** took the cash into the store, the employee told her the contracted had been voided because she did not bring the cash into the store with in 24 hours. Which was incorrect, **** signed another contract and payment arraignment for the same package. **** also purchased other services a couple days later. The employee has been reprimanded and this issue has been resolved. I have up loaded copies of the payment arraignments and contracts **** signed. As a company with signed contracts we could have held **** to all three contracts because she signed the payment arraignment, as well as the contracts. Complaint: I signed a contact at the end of January (28th) 2013 purchasing $500 in services. I put down $100 as down payment. During my short time of signing the contract and observing poor professionalism of staff, I went directly home to read reviews of Forever Young online. Due to the huge amount of negative comments, I decided to cancel the contract by mailing in my written response and sending it to the Prineville location. I did not send it certified mail, however, I realized my mistake the following day and decided that I would call BOTH the Prineville and Scholls Ferry location to let them know of this. I left messages at both places, each employee told me that "the lady handling it was out on vacation," "Or not in at the moment". I offered to leave my email address, and explained my situation. A call was placed every week for two months and I never got a reply. On at least 5 occasions, I let the phone ring without voicemail ever picking up for a message to be left. Later, I find out that their offices moved from Prineville to Portland. At the end of March the prearranged fee of $45 was deducted out of my account. I called immediately to the Scholls Ferry location, explained that my contract should have been cancelled, They stated, "hold on let me see if we got your letter," I was put on hold for 5 minutes without them ever having asked my name, and then eventually the call disconnected. I was so angry, I decided to wait to contact them the next day where I finally spoke with "headquarters". She stated that they were not obligated to refund me due to not having the contract sent by certified mail. She asked if I wanted to transfer services, or sell off my package to someone else. Another fee was deducted out of my account two days ago, it is just not right to have to go through this.

Desired Settlement: I am asking a for a full refund of $190.00

The way that I feel towards the business is secondary to the fact that I am an easy example of the harsh customer service experiences that multiple other people have been going through. I know this letter might sound condescending, no disrespect for those who are just doing their jobs, but really.... Just think about this letter. I have not had to cancel many contracts in my life and play it fair when mistakes are made. However, the length of time, the poor contact over two months, and the time consuming options that are available for me to be refunded is much more than anyone should deal with, especially in my case when there is no proof of what happened to the missing mailed cancellation contract. My question is, How come I have no issues mailing letters, and other people receiving them, ever? We live in a pretty reliable world when it comes to the modern day postal system. Perhaps this suggests a little disorganization on the business's part. I get it...Forever Young wants to protect themselves from random cancellations and people changing their minds about receiving services. To me, $190.00 in the grand scheme is not a lot of money to ask from a business that has multiple locations. I know based on what was reported in the evening news, that a woman was refunded $1500 for services she used. I have used nothing nor made any appointments since the day I signed....smaller businesses have refunded a lot more for the value of rebuilding reputation and keeping future customers happy. It is understandable that protecting company policy should be consistent. However, if I were a business that is actively trying to give themselves a * face lift * for the huge amount of poor and low result of services provided, an F rating from the BBB in Feb 2013, and horrible reviews on *****'s **** (a review site people take seriously).......I would work with people. Complaint: I have a serious grievance with the business Forever Young. I have heard of many people expressing concerns with their treatments. There is always a risk of signing up for "cosmetic" procedures...I get that and personally have not had issues with the training or use of machines by their technicians. My issue is what happens at the front end...their customer service. When they are trying to get you to sign up for service, they bend over backwards with promises and even fitting you in for appointments with the next day or two. The fact is Forever Young has horrible customer service. They are typically running behind, they move appointments without notifying you (I have print outs they gave me with actual appointment times), it is hard to get an appointment most of the time, and then they treat you as if it is your fault that they are backed up or overworked. I have had 5 occurrences in the last 1 1/2 years where they moved my appointment to an earlier or later time and never notified me of the change. There were also 2 occurrences in the last 2 years where they were not able to do my treatment because of not having the right technician and they never notified me until after I arrived and waited for my appointment. I also on at least 2 occasions have overheard them move another customers appointment to fit someone else in...and they never actually called the customer to confirm that. The icing on the cake was my appointment today that was scheduled at 9:45pm. First I was told that I was wrong, my appointment was not until 10pm. I showed my confirmation printout that showed I was scheduled at 9:45pm. I was then told a couple of times the tech would be with me soon. At 10:17pm, over 30 minutes after my appointment was to begin, I was told there was no tech and I couldn't get treated. This is beyond frustrating and unacceptable; my time is important. While the services may be well priced, the customer service is beyond horrible. Had I known all these problems, I would have paid a little more to have some more consistency. I have patience, but this has happened one too many times for my liking. I don't know how they can still be in business and why despite a number of complaints from myself and other customers, this behavior continues. Something needs to change.

Desired Settlement: I don't want money, I don't think they will compensate me for the waste of my time. I want the service issues to be corrected so I can get treatment on time, as expected.

Complaint: This company does not stand by its products they are not working, and when I try and discuss it with them they try and give me more products and treat me like Im dumb they have over charged my card and over drew my acct it took along time for them to put the money back in and did not credit the over drafts they occured. I have called cooperate and they just tell me that there products work. ***** and ***** are sharing my private chart, including what treatments I have been getting , if I have paid my payment and such to the public Including two friends of mine that they obviously didnt know who knew me this is a direct violation of my Hippa and it should not be shared its against the law. They do not keep the charts the way they are to be kept.

Desired Settlement: I would Like my money back and compensation from all the time I have missed from work to attend these appts. and Also would like them to be taught about privacy and hippa practices.

Complaint: After a high pressure sales to purchase the Qswitch laser the laser is never available. I have schedule 4 appointments in advance and each time the day of the appointment I receive a call that they will need to reschedule.

Desired Settlement: I require the company return my entire contract amount to my bank account and restore my original package that was bought 2 years prior.

Complaint: I bought a package for $800.00 dollars early November 2012 for 3 areas of Velashape (my tummy, my bleg, and my love handles) In December (on Black Friday) they told me to come in because they were basically giving packages away compared to what they would normally be priced at. I went in that morning and bought another package for $1300.00 or so (hair removal on three areas and 12 more treatments of Velashape. I made it to all my appointments weekly and drank lots of water. After two months and 9 treatments into it I realized that my before and after pictures were identical (I also gained 5 lbs during that time cause unknown) I went in and spoke with ***** who assured me I would see results. I waiting and booked more appointments (also due to the fact that they were understaffed and overbooked they could only fit me in every OTHER week (twice a month). I was not happy about that because I read that Velashape needs to be done every week in order to work correctly) but they told me they couldn't do anything about that. So after another two months I still had not noticed any difference so I went to talk with ***** (again) she told me they just got a new machine (accent) in and that "would work much better the Velshape" I was frustrated but I agreed to try it "for three treatments in place of my Velashape." A month and a half later my results are still identical. During my five months as a client at Forever Young my appointments have not been written down and had to be rescheduled at a later time, (normally a week or more later)between 9:05 am to 4:30 PM I called 15 times on 03/23/13 because I had to reschedule my appointment due to a work meeting and no one returned my calls OR picked up the phone (not even an answering machine) ***** also told me that due to new hires my Velshape/Accent and Laser hair removal appointments were running about 20 mins or less each and they should be longer than that in order to see results. ***** said we can try pushing my appointments to an hour and a half in order to see results. The problem is I have ovarian cysts and cannot due an hour and a half each appt. I been wasting my time for over five months because Forever Young did not uphold the contract to me as a customer to schedule my appointments on time, and update my chart. I would go there an they did not even have me down at all! My chart has not been updated since January last time I spoke with ***** (March 2013) ***** and I looked at it. I keep records of all my appointments on my Calendar on my phone but the fact that Forever Young has not updated my chart is shocking. For hair removal to work it needs to be done every so often (body and face is different weeks) and due to the appointments being overly booked and my chart not being updated the treatments are not working. I have been asking for someone to assist me in bringing my monthly cost down since they have not been professional in their services. I also was burned on one of my sessions of Velashape (I now have a scar on my back with three brown holes from the head of the machine) when I told ***** that I was burned I was informed that the machine does not burn you, I however still (4 1/2 months later) the scar remains. I spoke to ***** on 03/26/13 via phone and asked her that I need to adjust my monthly payments to remove the price of the Velshape and the Accent. I have spent a lot of time and money and due to girls leaving and being under staffed I am not happy with the services. I have been trying to get a hold of someone at the main head quarters to discuss my issues and get it resolved however they have not contacted me and when I finally spoke to someone they said they do "not" refund any money. I have been trying to explain my case with them but now they are not returning any of my calls. Desired Settlement: I need the payments to stop on both cards. Considering the horrible service I want a full refund for my time and the money I spent and wasted the past 5 and a half months. My bruise on my back is still there and will be until I get it professionally removed. I have been given the run around for the past few weeks on who to contact regarding my monthly payments and due to the fact that they are famous for money fraud I need this to be resolved before the 12th and the 14th of April which is when they take out my payments. I am afraid they will take out more then they are suppose to due to the fact that I have been complaining.

I accepted this deal and I will continue to tan and get hair removal as long as they do not charge my cards.
